血清和肽素水平测定在心力衰竭患者诊断和预后评价中的价值

The Value of Serum and Copeptin Levels in the Diagnosis and Prognostic Evaluation of Patients with Heart Failure

摘要 目的探讨血清和肽素水平测定在心力衰竭患者诊断和预后中的价值。方法选取2018年7月~2020年7月我院诊治的190例心力衰竭患者设为观察组,另选取同期我院体检健康者190名作为对照组,比较两组血清CPT、BNP水平及不同心功能分级和不同预后心力衰竭患者CPT和BNP水平,并分析不同指标诊断心力衰竭的诊断效能及CPT与心功能分级的相关性。结果观察组CPT、BNP水平均高于对照组,差异有统计学意义(P<0.05);心功能Ⅳ级心力衰竭者CPT、BNP均高于心功能Ⅱ级、Ⅲ级心力衰竭者,且Ⅲ级高于Ⅱ级,差异有统计学意义(P<0.05);治疗有效者CPT、BNP水平均低于无效者,差异有统计学意义(P<0.05);CPT诊断心力衰竭敏感度高于BNP、CPT联合BNP,特异度低于BNP、CPT联合BNP(P<0.05);CPT与心功能分级呈正相关(P<0.05)。结论血清和肽素水平测定可较好的预测心力衰竭患者病情严重程度,并且心功能分级与血清和肽素密切相关,可作为心力衰竭患者的早期诊断、预后评估的指标。 Objective To explore the value of serum and copeptin levels in the diagnosis and prognosis of patients with heart failure.Methods 190 patients with heart failure diagnosed and treated in our hospital from July 2018 to July 2020 were selected as the observation group.In addition,190 healthy people in our hospital during the physical examination were selected as the control group to compare the serum CPT and BNP levels and the CPT and BNP levels of patients with heart failure with different cardiac function grades and different prognosis and analyze the diagnostic efficacy of different indicators in the diagnosis of heart failure and the correlation between CPT and cardiac function classification.Results The CPT and BNP levels of the observation group were higher than those of the control group,the difference was statistically significant(P<0.05);CPT and BNP in patients with heart function gradeⅣheart failure were higher than those with heart function gradeⅡand gradeⅢheart failure,and gradeⅢwas higher than gradeⅡ,the difference was statistically significant(P<0.05);The CPT and BNP levels of the effective treatment were lower than those of the ineffective,the difference was statistically significant(P<0.05);The sensitivity of CPT to diagnose heart failure was higher than that of BNP,CPT combined with BNP,and the specificity was lower than that of BNP,CPT combined with BNP(P<0.05);CPT was positively correlated with cardiac function classification(P<0.05).Conclusion The determination of serum and copeptin levels can better predict the severity of heart failure in patients with heart failure,and the classification of heart function is closely related to serum and copeptin,and can be used as an indicator for early diagnosis and prognostic evaluation of heart failure patients.
