产前系统评分对提高剖宫产后再妊娠阴道分娩成功率的效果观察

Effect of prenatal system score on improving the success rate of vaginal delivery after cesarean section

目的观察产前系统评分对提高剖宫产后再妊娠阴道分娩成功率的效果。方法选取2015年1月至2018年12月本院收治的54例行阴道分娩产妇作为研究对象,前次分娩采用剖宫产,本次实施阴道分娩成功产妇作为观察组(n=36),失败产妇作为对照组(n=18)。按照本院编制的三水平10分制7指标评分标准,比较两组评分情况,评价7指标与剖腹产后再妊娠阴道分娩成功的相关性。结果两组产妇年龄、体重指数、瘢痕子宫下段厚度、剖宫产与阴道分娩间隔时间、Bishop评分、胎儿体质量、产程的三水平10分制评分比较差异有统计学意义(P<0.01)。阴道分娩产妇总评分与孕妇年龄、体重指数、瘢痕子宫下段厚度、剖宫产与阴道分娩间隔时间、产程、Bishop评分、胎儿体质量评分等级均显著相关(P<0.05)。结论自制三水平10分制对孕妇年龄、体重指数、瘢痕子宫下段厚度、剖宫产与阴道分娩间隔时间、Dishop、产程进行系统评分对提高剖宫产后再妊娠阴道分娩成功率有一定的预测价值。

Objective To observe the effect of prenatal system score on improving the success rate of vaginal delivery after cesarean section.Methods From January 2015 to December 2018,54 women with vaginal delivery in our hospital were selected as the research subjects.The success-ful women with cesarean section in the previous delivery were selected as the observation group(n=36),and the unsuccessful women were selected as the control group(n=18).According to the three-level 10-point system and 7-index scoring standard prepared by our hospital,the scores between the two groups were compared to evaluate the correlation between the 7-index and the success of vaginal delivery after caesarean section.Results There was statistically significant difference between the two groups in age,body mass index,thickness of lower segment of scar uterus,interval time be-tween cesarean section and vaginal delivery,Bishop score,fetal body weight,and 10-point scale score of the three levels of labor(P<0.01).The total score of vaginal delivery was significantly correlated with pregnant women's age,body mass index,thickness of lower segment of scar uterus,inter-val time between cesarean section and vaginal delivery,labor course,Bishop score and fetal body mass score(P<0.05).Conclusion The self-made three-level 10-point system has certain predictive value in predicting the success rate of vaginal delivery after cesarean section by systematically scor-ing pregnant women's age,body mass index,thickness of lower segment of scar uterus,interval time between cesarean section and vaginal delivery,Dishop and labor course.
