腹腔镜胆囊切除胆道探查术对胆囊结石合并胆总管结石患者康复时间及出血情况的影响

Effect of laparoscopic cholecystectomy and biliary tract exploration on recovery time and bleeding in patients with cholecystolithiasis complicated with common bile duct stones

目的探究腹腔镜胆囊切除胆道探查术对胆囊结石合并胆总管结石患者康复时间及出血情况的影响。方法选取2018年11月至2019年10月本院收治的79例胆囊结石合并胆总管结石患者作为研究对象,采用随机数字表法分为对照组(采用传统开腹手术,n=39)和观察组(采用腹腔镜胆囊切除胆道探查术,n=40),比较两组临床疗效、手术指标、结石清除率、视觉模拟(VAS)评分和并发症。结果观察组治疗总有效率、结石清除率均高于对照组,术中出血量少于对照组,切口长度、住院时间、术后排气时间均短于对照组,VAS评分、并发症发生率低于对照组,差异均有统计学意义(P<0.05)。结论在胆囊结石合并胆总管结石患者中应用腹腔镜胆囊切除胆道探查术,临床疗效显著,手术指标改善明显,结石清除率高,疼痛程度轻,安全性较高。

Objective To explore the effect of laparoscopic cholecystectomy and bile duct exploration on the recovery time and bleeding of pa-tients with cholecystolithiasis complicated with choledocholithiasis.Methods 79 patients with cholecystolithiasis and choledocholithiasis from No-vember 2018 to October 2019 were selected and they were randomly divided into control group(traditional open surgery,n=39)and observation group(laparoscopic cholecystectomy and bile duct exploration,n=40).The clinical efficacy,surgical indicators,stone clearance rate and visual ana-logue scale(VAS)and complications between the two groups were compared.Results The total effective rate and stone clearance rate of the obser-vation group were higher than those of the control group,the intraoperative blood loss of the observation group was less than that of the control group,the incision length,hospitalization time and postoperative exhaust time of the observation group were shorter than those of the control group,the VAS score and the complication rate of the observation group were lower than those of the control group,the difference was statistically signifi-cant(P<0.05).Conclusion The application of laparoscopic cholecystectomy and bile duct exploration in patients with cholecystolithiasis and cho-ledocholithiasis has significant clinical effect,obvious improvement of operation indexes,high stone clearance rate,light pain and high safety.
