宫颈黏膜受累及宫颈钙化斑对宫颈癌的超声诊断价值

Ultrasound Diagnostic Value of Cervical Mucosa Involvement and Cervical Calcification Spot in Cervical Carcinoma

目的探讨宫颈黏膜受累及宫颈钙化斑对宫颈癌的超声诊断价值,提高阴道超声对宫颈癌与宫颈肌瘤的鉴别诊断准确率。方法回顾性分析超声检查发现的宫颈肿块患者125例,其中67例宫颈癌,58例宫颈肌瘤,均经病理证实。阴道超声观察肿块位置、大小、回声、钙化、边界、形态、血流的超声表现,使用统计学软件SPSS 22.0分析数据。结果宫颈肿块的大小与回声,在宫颈癌与宫颈肌瘤组间的差异无统计学意义(均P>0.05)。宫颈肿块的位置、钙化、边界、形态与血流,在宫颈癌与宫颈肌瘤组间的差异有统计学意义(均P<0.05)。与肿块位于宫颈前壁相比,宫颈黏膜受累发生宫颈癌的风险是宫颈肌瘤的75.429倍。与无钙化相比,宫颈肿块伴砂砾点状或斑点状钙化发生宫颈癌的风险是宫颈肌瘤的6.562倍。结论宫颈黏膜受累及宫颈钙化斑对于宫颈癌的超声诊断具有一定的提示意义。

Objective To investigate the value of cervical mucosa involvement and cervical calcification spot in the ultrasound diagnosis of cervical carcinoma,and to improve the accuracy of vaginal ultrasound in the differential diagnosis of cervical carcinoma and cervical myoma.Methods Totally,125 cases of cervical masses found by ultrasound were retrospectively analyzed.Among them,67 cases were cervical carcinoma and 58 cases were cervical myoma confirmed by pathological examination.The location,size,echo,calcification,boundary,morphology,blood flow of the masses were observed by vaginal ultrasound.The statistical software SPSS 22.0 was used to analyze the data.Results There were no significant differences in the size and echo of cervical mass between cervical carcinoma group and cervical myoma group(P>0.05).There were significant differences in location,calcification,boundary,morphology and blood flow between cervical carcinoma and cervical myoma group(P<0.05).Compared with the mass in the anterior wall,the risk of cervical carcinoma for cervical mass with cervical mucosa involvement is 75.429 times that of cervical myoma.Compared with non-calcification,the risk of cervical carcinoma for cervical mass with granular or spot-like calcificationis was 6.562 times that of cervical myoma.Conclusion Cervical mucosa involvement and cervical calcification spot have certain significance for ultrasound diagnosis of cervical carcinoma.
