以胜任力为导向的分阶段培养在心胸外科住院医师规范化培训中的应用效果

Application effect of competency-oriented phased training in the standardized training of cardiothoracic surgery residents

目的探讨以胜任力为导向的分阶段培养在心胸外科住院医师规范化培训中的应用效果。方法选取2018年1月—2019年12月赣州市人民医院心胸外科住院医师规范化培训学员30名作为研究对象,随机摸球法分为两组,对照组(15名)采用传统培训,观察组(15名)采用以胜任力为导向的分阶段培养。对比两种不同培养方式下学员学习效果。结果两组学员培训后各项能力评分均高于培训前,差异有统计学意义(P<0.05);观察组学员培训后查房能力、管理医疗档案能力等评分均高于对照组,差异有统计学意义(P<0.05);两组学员培训前职业能力、独立处理疾病能力等评分比较,差异无统计学意义(P>0.05)。两组学员培训前理论考核和技能考核成绩比较,差异无统计学意义(P>0.05);观察组学员培训后理论考核和技能考核成绩均高于对照组,差异有统计学意义(P<0.05);两组学员培训后理论考核和技能考核成绩均高于培训前,差异有统计学意义(P<0.05)。结论心胸外科住院医师规范培训时采用胜任力为导向的分阶段培养,不仅可提升学员独立处理疾病能力、职业能力,同时也能提升理论成绩和操作技能,具有应用价值。

Objective To explore the application effect of competency-oriented phased training in the standardized training of cardiothoracic surgery residents.Methods From January 2018 to December 2019,30 trainees from the standardized training of cardiothoracic surgery residents in Ganzhou People′s Hospital were selected as the research subjects.Random touched balls were used to divide them into two groups.The control group(15 cases)received traditional training and the observation group(15 cases)received competency-oriented phased training.The learning effect of students under two different training methods was compared.Results The scores of each ability of the normalized training trainees in the two groups after training were higher than those before training,and the differences were statistically significant(P<0.05).After training,the scores of ward round ability and management of medical files in the observation group were all higher than in the control group,the differences were statistically significant(P<0.05).There were no statistically significant differences in the scores of trainees before training between the two groups,such as professional ability and ability to deal with diseases independently(P>0.05).There were no statistically significant differences in theoretical assessment and skill assessment of trainees before training between the two groups(P>0.05).After training,the performances of the trainees in theoretical assessment and skill assessment in the observation group were higher than those in the control group,and the differences were statistically significant(P<0.05).After training,the performances of the trainees in theoretical assessment and skill assessment in the two groups were higher than those before training,and the differences were statistically significant(P<0.05).Conclusion The competency-oriented phased training in the standardized training of cardiothoracic surgery residents not only improves the students′ability to deal with diseases independently and professionally,but also improves theoretical performance and operational skills,which has application value.
