2型糖尿病合并肺部感染患者的感染特点及影像学表现

Infection Characteristics and Imaging Findings in Patients with Type 2 Diabetes Mellitus Complicated with Lung Infection

目的:观察2型糖尿病合并肺部感染患者的感染特点以及相关影像学检查表现。方法:共有80例患者被纳入此次研究,均是本院2018年1月~2020年3月收治的2型糖尿病合并肺部感染患者。所有患者均接受病原微生物检查和CT检查,便于临床观察患者的感染特点和影像学表现。结果:80例患者共分离出74株病原菌,主要以格兰阴性菌为主;格兰阴性菌种肺炎克雷伯菌占比较高,革兰阳性菌中肺炎链球菌占比较高,真菌中白色假丝酵母菌占比较高;80例患者CT检查出,肺炎患者占比较高72.50%,主要以大叶性肺炎为主;真菌感染患者中肺曲菌病占比较高;不同类型肺部感染的CT影像学表现不尽相同。结论:2型糖尿病患者易合并肺部感染,以肺炎链球菌为主,运用CT检查能够更好地发现患者感染类型,便于临床进行准确诊断,对于改善患者预后十分重要。

Objective:To observe the characteristics of infection and related imaging findings in patients with type 2 diabetes and lung infection.Methods:A total of 80 patients were included in this study,all of whom were patients with type 2 diabetes and pulmonary infection admitted to our hospital from January 2018 to March 2020.All patients receive pathogenic microbial examination and CT examination,which is convenient for clinical observation of the patient’s infection characteristics and imaging findings.Results:A total of 74 strains of pathogenic bacteria were isolated from 80 patients,mainly Gram-negative bacteria;Gram-negative bacteria Klebsiella pneumoniae accounted for a higher proportion,Gram-positive bacteria accounted for a higher proportion of Streptococcus pneumoniae,and the fungus was white.Candida bacteria accounted for a relatively high proportion;CT examination of 80 patients showed that patients with pneumonia accounted for 72.50%,mainly lobar pneumonia;patients with fungal infections accounted for a relatively high proportion of pulmonary aspergillosis;different types of lung infections CT imaging manifestations are not the same.Conclusion:Patients with type 2 diabetes are prone to pulmonary infections,mainly streptococcus pneumoniae.CT examination can better discover the type of infection in patients,facilitate accurate clinical diagnosis,and is very important for improving patient prognosis.
