电刺激生物反馈结合手法按摩治疗盆底肌筋膜疼痛综合征临床分析

Clinical analysis of biofeedback of electrical stimulation combined with manual massage in the treatment of myofascial pain syndromeofpelvic floor

目的探讨电刺激生物反馈及手法按摩对于盆底肌筋膜疼痛综合征的临床疗效。方法选择64例盆底肌筋膜疼痛综合征的患者。随机分为观察组和对照组,每组患者32例,观察组采取电刺激生物反馈结合手法按摩的方法进行治疗,对照组仅用电刺激生物反馈治疗,比较2组的治疗效果。结果2组患者治疗后主观疼痛VAS评分、盆底肌筋膜压痛VAS评分、盆底肌Glazer评估前后静息电位均较治疗前下降,差异有统计学意义(P<0.05);其中主观疼痛VAS评分及盆底肌筋膜压痛VAS评分,观察组较对照组下降明显,差异有统计学意义(P<0.05);盆底肌Glazer评估前后静息电位观察组下降较对照组明显,但差异无统计学意义(P>0.05)。结论电刺激生物反馈及手法按摩对于盆底肌筋膜疼痛综合征都是安全有效的治疗方法,相比单纯电刺激生物反馈治疗,辅助手法按摩治疗后患者的主观疼痛及局部触诊疼痛缓解均更为明显,值得临床推广应用。

Objective To investigate the clinical effect of biofeedback of electrical stimulation and manual massage on myofascial pain syndrome of pelvic floor.Methods A total of 64 patients with myofascial pain syndrome of pelvic floor were randomly divided into the observation group and the control group(n=32).The observation group was treated with biofeedback of electrical stimulation combined with manual massage,while the control group was treated with biofeedback of electrical stimulation only.The therapeutic effects of the two groups were compared.Results After treatment,the VAS score of subjective pain ratings,VAS score of pelvic floor myofascial tenderness,and resting potential of pelvic floor muscles were all decreased in two groups compared with before treatment,and the differences were statistically significant(P<0.05);among them,the VAS score of subjective pain rating and VAS score of pelvic floor myofascial tenderness in the observation group decreased significantly compared with the control group(P<0.05);the resting potential of pelvic floor muscles in the observation group decreased significantly compared with the control groupbefore and after Glazer evaluation,but the difference was not statistically significant(P>0.05).Conclusion Both biofeedback of electrical stimulation and manual massage are safe and effective methods for the treatment of myofascial pain syndromeof pelvic floor.Compared with biofeedback of electrical stimulation alone,the subjective pain and localized pain on palpation of patients after auxiliary manual massage treatment are more significantly relieved,which is worthy of clinical promotion and application.
