胫骨横向骨搬移联合四妙勇安汤在糖尿病足中的应用效果分析

Analysis of the Application Effect of Tibial Transverse Transport Combined with Simiaoyong’an Decoction in Treating Diabetic Foot

目的分析胫骨横向骨搬移联合四妙勇安汤治疗糖尿病足的临床效果。方法选取2018年2月至2019年10月河南省洛阳正骨医院收治的40例糖尿病足患者作为研究对象,按照随机数表法将其随机分为研究组与对照组,每组20例,研究组患者采用胫骨横向骨搬移联合四妙勇安汤治疗,对照组患者单纯采用胫骨横向骨搬移治疗,对比两组患者并发症发生情况、创面愈合时间、治疗前后足背动脉血流速度及治疗满意度。结果研究组患者并发症发生率为5.0%,明显低于对照组患者的并发症发生率35.0%(χ^(2)=3.906,P=0.048);最终两组患者创面均完全愈合,其中研究组患者创面愈合时间为(25.2±3.8)d,明显短于对照组患者的创面愈合时间(33.3±5.1)d(t=5.664,P<0.001);治疗4周后,两组患者足背动脉血流速度均明显加快,其中研究组患者足背动脉血流速度为(35.1±0.8)cm/s,明显快于对照组患者的足背动脉血流速度(30.2±2.7)cm/s(t=7.782,P<0.001);研究组患者中十分满意11例、较为满意8例、不满意1例,明显优于对照组患者中十分满意7例、较为满意5例、不满意8例(Z=-2.023,P=0.043)。结论与单纯应用胫骨横向骨搬移相比,胫骨横向骨搬移联合四妙勇安汤可明显加快糖尿病足患者的足背动脉血流速度,缩短创面愈合时间,减少并发症发生率,提高治疗效果,增加患者满意度。

Objective To analyze the clinical efficacy of tibial transverse transport combined with Simiaoyong’an decoction in treating diabetic foot.Methods Forty patients with diabetic foot,admitted to Luoyang Orthopedic-Traumatological Hospital of Henan Province from February 2018 to October 2019,were selected as research subjects,and they were divided,according to the random number table,into a study group and a control group,with 20 cases in each group.Patients in the study group were treated with tibial transverse transport combined with Simiaoyong’an decoction,whereas patients in the control group with tibial transverse transport alone.The complication occurrence,wound healing time,foot dorsal artery flow velocity before and after treatment,and patients’satisfaction with treatment were compared between the two groups.Results The occurrence rate of complication in the study group was 5.0%,significantly lower than 35.0% in the control group(χ^(2)=3.906,P=0.048).In the end,the wounds of patients in the two groups were all healed completely,and the wound healing time in the study group was(25.2±3.8)d,being significantly shorter than(33.3±5.1)d in the control group(t=5.664,P<0.001).After 4 weeks of treatment,the foot dorsal artery flow velocities in the two groups all became markedly faster,and the velocity in the study group was(35.1±0.8)cm/s,being significantly faster than(30.2±2.7)cm/s in the control group(t=7.782,P<0.001).Among the patients in the study group,11 patients were very satisfied with the treatment,8 patients were satisfied,and 1 was dissatisfied,while in the control group 7 patients were very satisfied with the treatment,5 were satisfied,and 8 were dissatisfied,and the satisfaction of patient towards the treatment in the study group was much higher than that in the control group(Z=-2.023,P=0.043).Conclusion Compared with simple application of tibial transverse transport,the combined application of tibial transverse transport with Simiaoyong’an decoction in treating diabetic foot can significantly accelerate the flow velocity of dorsal artery of foot,shorten wound healing time,reduce incidence of complications and improve treatment effect,thus realizing a much high degree of patients’satisfaction.
