细菌培养与尿沉渣分析仪在尿路感染诊断中的应用意义

摘要 目的:探讨细菌培养与尿沉渣分析仪在尿路感染诊断中的应用价值。方法:选择2017年6月~2019年6月本院收治的疑似泌尿系统疾病患者102例尿液样本为研究对象,均行细菌培养与定量全自动尿沉渣分析仪检测,比较两组方法检出率,以细菌培养为标准,评价尿沉渣分析仪的准确率、特异度。结果:尿液培养细菌检测阳性率27.45%与尿沉渣分析仪检测阳性率24.51%比较无统计学意义,P>0.05;28份尿液培养细菌阳性标本中包含大肠埃希菌13份,肺炎克雷伯菌2份,变形杆菌1份,铜绿假单细胞菌3份,鲍曼不动杆菌1份,葡萄球菌3份,真菌5份。以尿液培养结果为参考标准,尿沉渣分析仪检测敏感度68.86%,特异度91.89%,准确率85.29%,阳性预测值76.00%,阴性预测值88.31%。结论:在尿路感染诊断中采用尿沉渣分析仪检测具有较高价值,具有快速筛查优势,但无法作为确诊方法,仍需细菌培养确诊。 Objective:To explore the value of bacterial culture and urine sediment analyzer in the diagnosis of urinary tract infection.Methods:102 patients with suspected urinary system diseases treated in our hospital from June 2017 to June 2019 were selected as the research data.A total of 102 urine samples were tested by bacterial culture and quantitative automatic urine sediment analyzer.The detection rate of the two groups was compared.The accuracy and specificity of urine sediment analyzer were evaluated by using bacterial culture as the standard.Results:The positive rate of urine culture bacteria was 27.45%,compared with 24.51%by urine sediment analyzer,P>0.05;28 positive samples of urine culture bacteria included 13 Escherichia coli,2 Klebsiella pneumoniae,1 Proteus,3 Pseudomonas aeruginosa,1 Acinetobacter baumannii,3 Staphylococcus and 5 fungi.The sensitivity of urine sediment analyzer was 68.86%,specificity was 91.89%,accuracy was 85.29%,positive predictive value was 76.00%,negative predictive value was 88.31%.Conclusion:The use of urine sediment analyzer in the diagnosis of urinary tract infection is of high value and has the advantage of rapid screening,but it can not be used as a diagnostic method,and still needs the diagnosis of bacterial culture,and both methods need to be popularized and applied.
