早期局限性肾癌患者肾周脂肪粘连的预测因素

Predictive factors of adherent perinephric fat in patients with early localized renal cell carcinoma

目的探讨早期局限性肾癌患者肾周脂肪粘连(APF)的临床预测因素。方法回顾性分析2018年1月至2020年5月徐州医科大学附属医院收治的132例经腹腔镜下肾部分切除术(LPN)治疗的早期局限性肾癌患者的病例资料,根据术中观察、手术记录将患者分为APF组和非APF组,采用多因素分析APF的危险因素。结果高体质指数(BMI)、男性、高龄、合并高血压、糖尿病、高脂血症、高梅奥粘连概率(MAP)评分是APF的危险因素(P<0.05);多因素Logistic回归分析显示,MAP评分是早期局限性肾癌患者APF的独立预测因素(OR=2.717,P<0.05)。结论术前应用MAP评分系统充分评估肾周脂肪的粘连情况,可以制定合理的术前计划,提高手术治疗效果。

Objective To investigate the predictive factors of adherent perinephric fat(APF)in patients with early localized renal cell carcinoma(RCC).Methods The clinical data of 60 patients with early localized RCC treated with laparoscopic partial nephrectomy(LPN)in the Affiliated Hospital of Xuzhou Medical University during Jan.2018 and May 2020 were retrospectively analyzed.The patients were divided into APF group and non-APF group.The risk factors of APF were screened with multivariate Logistic regression analysis.Results High body mass index(BMI),male,advanced age,hypertension,diabetes,hyperlipidemia and high MAP score were risk factors of APF(P<0.05).Multivariate Logistic regression analysis showed that high MAP score was an independent predictor of APF(OR=2.717,P<0.05).Conclusion Preoperative application of MAP scoring system to evaluate APF and formulation of a reasonable plan can improve the outcomes of surgical treatment.
