Understanding Heart Failure from TCM Thinking

中医形象思维认为,心象火,主温通,属阳中之阳。心衰之为病,必先损其心气,渐及心阴、心阳;中医天地人一体思维认为,人不是孤立的,无时无刻不受所处的自然、社会环境的影响。诊治心力衰竭,应将其置放于天、地、人三大要素中去分析衡量,寻找不同季节心衰发病率、死亡率出现差异的本质和规律;中医辨证思维强调,对心力衰竭进行药物干预的同时,要抓住心气是心阴、心阳之根,心气耗损是导致心阴、心阳失调的根源这一关键点,通过补益心气,恢复心的生理机能,使心阴、心阳之间得以自和与平衡;中医类推思维以益气活血为治疗心衰的基本大法,强调偏阳虚则辅以温阳,偏阴虚则佐以养阴,合并水饮、痰浊则利水化痰。

TCM Image thinking holds that the Heart is like fire,which governs warming,and belongs to the Yang within Yang.When heart failure(HF)happens,the heart Qi was damaged first,and gradually affects the heart Yin and heart Yang.TCM Heaven-earth-human harmony thinking believes that human is not isolated,and affected the surrounding natural and social environment constantly.In the diagnosis and treatment of heart failure,it should be placed in the three elements of heaven,earth,and people to inspect and analyze and find the essence and pattern of the difference in the incidence and mortality of HF in different seasons.TCM dialectical thinking emphasizes that while carrying out drug interventions on HF,it is necessary to grasp the key point that heart Qi is the source of heart Yin and heart Yang,and heart Qi loss is the root cause of heart Yin and heart Yang disharmony.And by replenishing heart Qi and restoring the physiological function of the heart,the heart Yin and heart Yang can be reconciled and balanced.TCM analogical thinking using Qi replenishing and Blood activating as the basic method in treating HF,emphasizing that partial Yang deficiency should supplement with warming Yang,and partial Yin deficiency should supplement with Yin nourishing,while combined with edema,turbid phlegm should diuresis and resolving phlegm.
