组合式输尿管软镜联合钬激光治疗肾结石的临床效果及安全性

Clinical effect and safety of modular flexible ureteroscope combined with holmium laser in the treatment of renal calculi

目的探讨组合式输尿管软镜联合钬激光治疗肾结石的临床效果及安全性。方法选取本院2017年6月至2020年4月收治的78例肾结石患者作为研究对象,根据整群随机法将其分为试验组(39例,组合式输尿管软镜联合钬激光)与对照组(39例,经皮肾镜取石术)。比较两组的手术及住院情况、氧化应激指标、炎症指标及不良事件发生情况。结果试验组的住院时间、平均排石天数、手术时间均短于对照组,结石清除率高于对照组,术中出血量少于对照组(P<0.05)。手术后,试验组的SOD水平高于对照组,MDA水平低于对照组(P<0.05)。手术后,试验组的hs-CRP水平及WBC均低于对照组(P<0.05)。试验组的不良事件总发生率低于对照组(P<0.05)。结论组合式输尿管软镜联合钬激光可减少肾结石患者的氧化应激反应与炎症反应,提高结石清除率,降低不良事件发生率。

Objective To investigate the clinical effect and safety of modular flexible ureteroscope combined with holmium laser in the treatment of renal calculi.Methods A total of 78 patients with renal calculi admitted in our hospital from June 2017 to April 2020 were selected as the research objects and divided into experimental group(39 cases,modular flexible ureteroscope combined with holmium laser)and control group(39 cases,percutaneous nephrolithotomy)according to the cluster random method.The operation and hospitalization,oxidative stress indexes,inflammatory indexes and adverse events were compared between the two groups.Results The hospital stay,the average days of stone removal and the operation time in the experimental group were shorter than those in the control group,the stone clearance rate was higher than that in the control group,and the intraoperative blood loss was less than that in the control group(P<0.05).After operation,SOD level in the experimental group was higher than that in the control group,while MDA level was lower than that in the control group(P<0.05).After operation,the hs-CRP level and WBC in the experimental group were lower than those in the control group(P<0.05).The total incidence of adverse events in the experimental group was lower than that in the control group(P<0.05).Conclusion Modular flexible ureteroscope combined with holmium laser can reduce the oxidative stress and inflammatory reaction,improve the stone clearance rate and reduce the incidence of adverse events.
