血清脂蛋白相关磷脂酶A_(2)、脂质代谢与儿童阻塞性睡眠呼吸暂停低通气综合征严重程度的相关性分析

摘要 目的探讨血清脂蛋白相关磷脂酶A_(2)(Lp-PLA_(2))、脂质代谢与儿童阻塞性睡眠呼吸暂停低通气综合征(OSAHS)疾病严重程度的关系。方法选取2018年1月至2020年10月于郑州大学第一附属医院住院治疗的OSAHS患儿151例。根据多导睡眠监测仪(PSG)记录的呼吸暂停低通气指数(AHI),将收集的OSAHS患儿分为轻度组、中度组、重度组3组,收集同一时期的50例体检的健康儿童作为对照组。对4组儿童进行全自动酶联免疫分析仪检测外周血Lp-PLA_(2)及甘油三酯(TG)、总胆固醇(TC)、低密度脂蛋白(LDL)、高密度脂蛋白(HDL)。对4组儿童外周血Lp-PLA_(2)、LDL、HDL、TG、TC进行组间比较;对OSAHS患儿外周血Lp-PLA_(2)、LDL、HDL、TG、TC与AHI进行相关性分析。结果轻度组、中度组、重度组外周血Lp-PLA_(2)、LDL、TG、TC高于对照组(P<0.05);轻度组、中度组、重度组外周血HDL低于对照组(P<0.05);重度组外周血Lp-PLA_(2)、LDL、TG、TC高于轻度组、中度组(P<0.05);重度组外周血HDL低于轻度组、中度组(P<0.05);中度组外周血Lp-PLA_(2)、LDL、TG高于轻度组(P<0.05);中度组外周血HDL、TC与轻度组比较差异无统计学意义(P>0.05)。OSAHS患儿外周血Lp-PLA_(2)、LDL、TG与AHI呈正相关(P<0.05),HDL与AHI呈负相关(P<0.05),OSAHS患儿外周血TC与AHI无相关性(P>0.05)。结论 Lp-PLA_(2)、LDL、TG及TC在OSAHS患儿外周血水平升高,随疾病严重程度升高而升高;HDL在OSAHS患儿外周血水平降低,随疾病严重程度的升高而减低。 Objective To study the relationship between serum lipoprotein-associated phospholipase A_(2)(Lp-PLA_(2)),lipid metabolism and the severity of Obstructive sleep apnea-hypopnea syndrome(OSAHS)in children.Methods A total of 151 children with OSAHS in the First Affiliated Hospital of Zhengzhou University from January 2018 to October 2020 were selected.According to the apnea hypopnea index(AHI)recorded by polysomnography monitor(PSG),the children with OSAHS were divided into three groups:mild group,moderate group and severe group.Fifty healthy children with physical examination during the same period were collected as the control group.Lp-PLA_(2),triglyceride(TG),total cholesterol(TC),low density lipoprotein(LDL)and high density lipoprotein(HDL)in peripheral blood were detected by automatic enzyme linked immunoassay.Lp-PLA_(2),LDL,HDL,TG and TC in the peripheral blood of children in 4 groups were compared among groups.The correlation among Lp-PLA_(2),LDL,HDL,TG,TC and AHI in peripheral blood of children with OSAHS were analyzed.Results Lp-PLA_(2),LDL,TG and TC in peripheral blood of mild,moderate and severe groups were higher than those of control group(P<0.05).Peripheral blood HDL in mild,moderate and severe groups was lower than that in control group(P<0.05).Lp-PLA_(2),LDL,TG,TC in peripheral blood of severe group were higher than those of mild group and moderate group(P<0.05).Peripheral blood HDL in severe group was lower than that in mild and moderate groups(P<0.05).Lp-PLA_(2),LDL and TG in peripheral blood of moderate group were higher than those of mild group(P<0.05).There were no significant difference between moderate group and mild group in peripheral blood HDL and TC(P>0.05).OSAHS patients peripheral blood Lp-PLA_(2),LDL,TG were positively correlated with AHI(P<0.05),and HDL was negatively correlated with AHI(P<0.05).Children with OSAHS had no relevance on peripheral blood TC with AHI(P>0.05).Conclusion The levels of Lp-PLA_(2),LDL,TG and TC in the peripheral blood of children with OSAHS increase,and increase with the increase of disease severity.The level of HDL in the peripheral blood of children with OSAHS decrease with the increase of the severity of the disease.
