摘要 目的探讨磁共振(MRI)不同扫描序列对脑小血管病伴微出血灶的评估价值。方法选择信阳市第一人民医院2018年8月至2020年5月收治的82例脑小血管病患者作为研究对象,所有患者入院当天均行MRI诊断,比较MRI不同扫描序列对脑小血管病伴微出血灶的阳性检出率、微出血灶检出数量及微出血灶的最大径线,分析脑小血管病伴微出血灶的MRI信号特点。结果 T_(1)加权成像(T_(1)WI)序列未检出微出血病灶,敏感加权成像(SWI)序列阳性检出率高于T_(1)加权成像(T_(1)WI)、T_(2)加权成像(T_(2)WI)、弥散加权成像(DWI)序列,DWI序列阳性检出率高于T_(1)WI、T_(2)WI,差异有统计学意义(P<0.05)。SWI序列检出微出血灶数量多于DWI序列,DWI序列检出微出血灶数量多于T_(2)WI序列,差异有统计学意义(P<0.05)。SWI序列最大径线大于T_(2)WI、DWI序列,DWI序列最大径线大于T_(2)WI序列,差异有统计学意义(P<0.05)。结论 T_(1)WI基本无法分辨微出血病灶,不适用该病的诊断,SWI优势明显,不仅可提高阳性检出率及病灶检出数量,还可确定病灶范围,有助于临床评估病情程度。 Objective To explore the value of different magnetic resonance imaging(MRI)scanning sequences in the evaluation of cerebral small vessel disease with microbleeds.Methods Eighty-two patients with cerebral small vessel disease admitted to the First People’s Hospital of Xinyang City from August 2018 to May 2020 were selected as the research subjects.All patients were diagnosed by MRI on the day of admission.The positive detection rate,the number of microbleeds and the maximum diameter of microbleeds in different MRI sequences on cerebral small vessel disease with microbleeds were compared.The MRI signal characteristics of cerebral small vessel disease with microbleeds were analyzed.Results No microbleeds was detected on T_(1)weighted imaging(T 1WI).The positive rate of sensitivity weighted imaging(SWI)was higher than that of T_(1)weighted imaging(T 1WI),T_(2) weighted imaging(T_(2)WI),diffusion weighted imaging(DWI)sequence.The positive rate of DWI sequence was higher than that of T 1WI and T_(2)WI,with statistical significant differences(P<0.05).SWI sequence detected more microbleeds than DWI sequence,and DWI sequence detected more microbleeds than T_(2)WI sequence,with statistical significant differences(P<0.05).The maximum diameter of SWI sequence was larger than that of T_(2)WI and DWI sequence,and the maximum diameter of DWI sequence was larger than that of T_(2)WI sequence,with statistical significant differences(P<0.05).Conclusion The microbleeds could not be distinguished on T 1WI,which is not suitable for the diagnosis of the disease.SWI has obvious advantages,it can not only improve the positive detection rate and the number of detected lesions,but also determine the scope of lesions,which is helpful for clinical evaluation of disease condition.
