35岁以下患者受精后第2天和第3天胚胎移植妊娠结局和子代出生情况

The outcome of embryo transfer and the birth condition of newborn in patients younger than 35-years-old at day 2 and day 3 after fertilization

目的探讨35岁以下患者进行受精后第2天(D2)和第3天(D3)胚胎移植的妊娠结局及子代出生情况。方法回顾性分析2017年1月-2019年12月上海市第一妇婴保健院辅助生殖医学科35岁以下患者行D2和D3胚胎移植的资料,其中在D2移植共158个周期,其中D3移植共3 601个周期。比较两组患者一般情况、妊娠结局及子代出生情况。结果 D2移植组患者基础FSH水平与D3移植组比较,差异有统计学意义(P<0.05);两组患者其他基本资料比较,差异均无统计学意义(均P>0.05)。两组患者临床妊娠率、着床率、多胎妊娠率、异位妊娠率及流产率比较,差异均无统计学意义(均P>0.05)。两组新生儿孕周、出生体质量、出生率、低体质量儿出生率及早产率比较,差异均无统计学意义(均P>0.05)。结论对于35岁以下患者,D2胚胎移植相对于D3胚胎移植不影响妊娠结果和子代出生情况,对于患者和生殖中心可以更加灵活地安排移植日期。

Objective To evaluate the pregnancy outcomes and the birth condition of newborn infants of D2 and D3 embryo transfer in patients younger than 35-years-old. Methods Retrospective analysis was made on the data of D2 and D3 embryo transfer for patients younger than 35-years-old who received D2 and D3 embryo transfer from January 2017 to December 2019 in Shanghai First Maternal and Infant Health Hospital. Among them,there were 158 cycles of D2 transfer and 3 601 cycles were D3 transfer. The general conditions,pregnancy outcome and offspring birth were compared between the two groups. Results The difference of basal FSH level between D2 transplantation group and D3 transplantation group was statistically significant(P< 0. 05);There was no statistical significance in the comparison of other basic information between the two groups(all P>0. 05). There were no significant differences in clinical pregnancy rate,implantation rate,multiple pregnancy rate,ectopic pregnancy rate and abortion rate between two groups(all P>0. 05). There were no significant differences in gestational age,birth weight,birth rate,birth rate of low birth weight and premature birth rate between the two groups(all P>0. 05). Conclusion For patients younger than 35-years-old,D2 embryo transfer does not affect pregnancy outcomes and offspring birth compared to D3 embryo transfer,and the date of transfer is more flexible for patients and fertility centers.
