减少杜冷丁用量辅助电针镇痛与常规杜冷丁用量在体外受精取卵术中的比较

Comparison of analgesic effects of low dosage of pethidine combined with electric acupuncture and routine dosage of pethidine in IVF oocyte retrieval

目的比较分析减少杜冷丁用量辅助电针镇痛与常规杜冷丁用量在体外受精-胚胎移植(IVF-ET)取卵术中的临床效果。方法选择桂林市妇幼保健院生殖中心2018年1月-2019年6月第一周期IVF-ET 108例患者作为研究对象,随机分为对照组53例和观察组55例,对照组患者在取卵术前5 min常规给予杜冷丁75 mg肌内注射,观察组患者在取卵术前5 min由中医师进行电针操作并同时予杜冷丁50 mg肌内注射,取卵术操作结束后继续10 min电针操作。结果两组患者临床资料中年龄、BMI、术前及术后血压、术中出血量、获卵数、移植日内膜、移植胚胎数及妊娠率差异无统计学意义(P>0.05)。两组患者疼痛评分、镇痛效果(满意)及术中不良反应(恶心、呕吐及头晕)差异无统计学意义(P>0.05)。观察组术后恢复与对照组比较差异有统计学意义(P<0.05)。结论减少杜冷丁用量辅助电针镇痛在IVF-ET取卵术中能达到与常规杜冷丁用量同样的镇痛效果,并且能减少杜冷丁肌内注射后的术中不良反应,患者术后恢复好。

Objective To compare the clinical effects of low dosage of pethidine combined with electric acupuncture and routine dosage of pethidine in in vitro fertilization-embryo transfer(IVF-ET) oocyte retrieval. Methods A total of 108 patients during the first cycle of IVF-ET were selected from Reproductive Center of Guilin Maternal and Child Health Care Hospital from January 2018 to June 2019,then they were randomly divided into control group(53 patients) and observation group(55 patients). The patients in control group were treated with intramuscular injection of pethidine(75 mg) at 5 minutes before oocyte retrieval;the patients in observation group were treated with intramuscular injection of pethidine(50 mg) and electric acupuncture at 5 minutes before oocyte retrieval,after oocyte retrieval,electric acupuncture was performed for 10 minutes. Results There was no statistically significant difference in age,BMI,preoperative and postoperative blood pressure,intraoperative blood loss,the number of oocytes retrieved,endometrium on transplantation day,the number of transplanted embryos,and pregnancy rate between the two groups(P > 0. 05). There was no statistically significant difference in pain score,analgesic effect(satisfaction),and intraoperative adverse reactions(nausea,vomiting,and dizziness) between the two groups(P>0. 05).There was statistically significant difference in postoperative recovery between the two groups(P < 0. 05). Conclusion Low dosage of pethidine combined with electric acupuncture can achieve the same analgesic effect as routine dosage of pethidine in IVF-ET oocyte retrieval,which can reduce intraoperative adverse reactions after intramuscular injection of pethidine,and the postoperative recovery of patients is good.
