老年糖尿病肾病患者心理困扰状况及其影响因素分析

Analysis of psychological distress and influencing factors in elderly patients with diabetic nephropathy

目的了解老年糖尿病肾病(DN)患者的心理困扰状况,分析可能存在的影响因素。方法整理四川大学华西医院2017年1月至2019年12月收治的在入院时均接受凯斯勒心理困扰量表(K10)评估的130例老年DN患者病历资料进行回顾性研究。比较不同资料特征的老年DN患者K10评分,分析老年DN患者心理困扰的相关影响因素。结果130例老年DN患者均存在不同程度心理困扰,K10中位得分为26.00(24.00,29.00)分。不同糖尿病病程、社会支持、负性情绪的老年DN患者K10评分比较,差异有统计学意义(P<0.05)。经多元线性回归分析结果显示,糖尿病(X1)、社会支持(X2)、负性情绪(X3)均可能是老年DN患者心理困扰状况的影响因素(P<0.05)。得到线性回归方程:Y=23.345+5.395X1+2.870X2+2.655X3。结论老年DN患者存在一定程度心理困扰,可能与患者糖尿病病程、社会支持水平、负性情绪有关,临床应重点关注上述因素,并给予相应干预。

Objective To understand the psychological distress of elderly patients with diabetic nephropathy(DN),and to analyze the possible influencing factors.Methods The medical records of 130 elderly patients with DN who were admitted to the West China Hospital of Sichuan University from January 2017 to December 2019 who were evaluated by the Kessler psychological distress scale(K10)at the time of admission were reviewed through retrospective research.The K10 scores of elderly DN patients with different data characteristics were compared,and the related factors that affect the psychological distress of elderly DN patients were analyzed.Results All 130 elderly patients with DN had different degrees of psychological distress,and the median K10 score was 26.00(24.00,29.00)points.The K10 scores of elderly DN patients with different diabetes course,social support,and negative emotions were compared,and the differences were statistically significant(P<0.05).The results of multiple linear regression analysis showed that course of diabetes(X1),social support(X2),and negative emotions(X3)might be the influencing factors of the psychological distress of elderly patients with DN(P<0.05).The linear regression equation was obtained:Y=23.345+5.395X1+2.870X2+2.655X3.Conclusion Elderly patients with DN have a certain degree of psychological distress,which may be related to the diabetes course,level of social support,negative emotions of patients.The clinic should focus on the above factors and give corresponding interventions.
