2型糖尿病患者血尿酸水平与肾功能的相关性研究

Correlation between serum uric acid and renal function in patients with diabetes mellitus type 2

目的分析探讨2型糖尿病患者血尿酸水平与肾功能的相关性。方法选取2016年1月至2020年8月安徽省芜湖市第一人民医院收治的2型糖尿病患者80例,根据血尿酸水平将其分为高尿酸血症(HUA)组(男性>420μmol/L,女性>360μmol/L,37例)和血尿酸正常(NUA)组(男性≤420μmol/L,女性≤360μmol/L,43例)。收集并比较两组临床一般资料及肾功能指标的差异。采用Pearson相关系数分析两组血尿酸水平与肾功能指标的相关性。结果HUA组血尿酸、尿素氮、血肌酐、肾功能不全发生率、尿微量白蛋白肌酐比值均高于NUA组,估算肾小球滤过率(eGFR)水平低于NUA组,差异均有统计学意义(均P<0.05)。Pearson相关分析结果显示,血尿酸与尿素氮(r=0.462)、血肌酐(r=0.527)、尿微量白蛋白肌酐比值(r=0.431)呈正相关,与eGFR(r=-0.475)呈负相关(P<0.05)。结论2型糖尿病患者血尿酸水平与肾功能显著相关,早期应重视2型糖尿病患者的血尿酸控制。

Objective To analyze the correlation between serum uric acid and renal function in patients with diabetes mellitus type 2.Methods A total of 80 patients with diabetes mellitus type 2 admitted to Wuhu First People’s Hospital,Anhui Province from January 2016 to August 2020 were selected.The patients were divided into hyperuricemia(HUA)group(male>420μmol/L,female>360μmol/L,37 cases)and normal uric acid(NUA)group(male≤420μmol/L,female≤360μmol/L,43 cases)according to their serum uric acid levels.The general clinical data and renal function indexes of the two groups were collected and compared.Pearson correlation coefficient was used to analyze the correlation between serum uric acid level and renal function indexes in the two groups.Results The levels of serum uric acid,urea nitrogen,serum creatinine,the incidence of renal insufficiency and urinary microalbumin creatinine ratio in HUA group were higher than those in NUA group,and the estimated glomerular filtration rate(eGFR)level was lower than that in NUA group,with statistical significance(P<0.05).Pearson correlation analysis showed that serum uric acid was positively correlated with urea nitrogen(r=0.462),serum creatinine(r=0.527),and urinary microalbumin creatinine ratio(r=0.431),and negatively correlated with eGFR(r=-0.475,P<0.05).Conclusion Serum uric acid is significantly correlated with renal function in patients with diabetes mellitus type 2,and more attention should be paid to the control of serum uric acid level in early phase.
