沙参桔梗汤联合泼尼松龙治疗放射性肺炎临床疗效观察

Clinical observation on Shashen Jiegeng Decoction combined with prednisolone on radiation pneumonia

目的观察沙参桔梗汤联合泼尼松龙治疗放射性肺炎(RP)临床疗效。方法将我院70例RP患者随机分为对照组与观察组各35例。对照组患者口服泼尼松龙治疗,观察组患者口服沙参桔梗汤联合泼尼松龙治疗。比较两组患者的临床总有效率、治疗前后患者的肺功能、血清炎症因子的水平和生存质量。结果观察组患者总有效率高于对照组(P<0.05);治疗后,两组患者1秒用力呼气容积(FEV1)、用力肺活量(FVC)、FEV1/FVC、肺活量(VC)、一氧化碳弥散量(DLCO)均增加,且观察组患者FEV1、FVC、FEV1/FVC、VC、DLCO均高于对照组(P<0.05);两组患者血清白介素-6(IL-6)、转化生长因子-1(TGF-1)、肿瘤坏死因子-(TNF-)水平均下降,且观察组患者血清IL-6、TGF-1、TNF-水平均低于对照组(P<0.05);两组患者生理、社交/家庭、情感、功能状况及总分均增加,且观察组均高于对照组(P<0.05)。结论沙参桔梗汤联合泼尼松龙治疗RP患者疗效显著,能明显地改善肺功能,降低炎症因子的表达,提高患者生存质量。

Objective To observe the efficacy of Shashen Jiegeng Decoction combined with prednisolone in treatment of radiation pneumonitis(RP).Methods A total of 70 patients with RP were randomly divided into control group and observation group,with 35 cases in each group.Control group took prednisolone,and patients in observation group took Shashen Jiegeng Decoction combined with prednisolone.The total effective rate,the lung function of the two groups before and after treatment,the level of serum inflammatory factors and the quality of life of the two groups were compared.Results The total effective rate of observation group was higher than control group(P<0.05);after treatment,one second(FEV1),forced vital capacity(FVC),FEV1/FVC,vital capacity(VC),and carbon monoxide diffusing capacity(DLCO)increased in both groups,FEV1,FVC,FEV1/FVC,VC,and DLCO in observation group were all higher than control group(P<0.05);the serum interleukin-6(IL-6),transforming growth factor-1(TGF-1),tumor necrosis factor-(TNF-)decreased in both groups,serum IL-6,TGF-1,and TNF-levels in observation group were lower than control group(P<0.05);the physiological,social/family,emotional,functional status and total scores of both groups increased,and were higher in observation group than those in control group(P<0.05).Conclusion Shashen Jiegeng Decoction combined with prednisolone is effective in treatment of RP,which can significantly improve lung function,reduce the expression of inflammatory factors,and improve the quality of life of patients.
