五苓散加减治疗痰浊内阻证老年肥胖代谢综合征临床疗效及对炎症指标的影响

Effect of Modified Wuling Powder on senile obesity metabolic syndrome of phlegm turbid internal obstruction syndrome and its influence on inflammatory indexes

目的观察五苓散加减治疗痰浊内阻证老年肥胖代谢综合征临床疗效及对炎症因子水平的影响。方法将我院100例痰浊内阻证老年肥胖代谢综合征患者随机分为观察组与对照组各50例。对照组行常规对症治疗以及饮食干预,观察组在对照组基础上服用中药方剂五苓散加减,对比两组治疗效果及对炎性因子水平的影响。结果观察组治疗总有效率明显高于对照组(P<0.05);治疗后,观察组腰围、体重、体重指数(BMI)、收缩压(SBP)、舒张压(DBP)、空腹血糖(FBG)、餐后0.5h血糖(PG 0.5h)、超敏C反应蛋白(hs-CRP)、白细胞介素(IL-6)、肿瘤坏死因子-(TNF-)明显低于对照组(P<0.05);但两组餐后1h血糖(PG1h)、餐后2h血糖(PG2h)及糖化血红蛋白(HbALC)比较无显著差异(P>0.05);两组总胆固醇(TC)、甘油三酯(TG)、低密度脂蛋白胆固醇(LDL-C)比较无显著差异(P>0.05),但观察组高密度脂蛋白胆固醇(HDLC)明显高于对照组(P<0.05);两组不良反应发生率比较无显著差异(P>0.05)。结论采用五苓散加减治疗痰浊内阻证老年肥胖代谢综合症疗效显著,能够显著改善患者血糖、血脂、血压水平,并对改善患者机体炎性反应状态有积极作用,具有临床应用及推广价值。

Objective To observe the clinical efficacy of Modified Wuling Powder in treatment of senile obesity metabolic syndrome of phlegm turbid internal obstruction syndrome and its influence on the level of inflammatory factors.Methods 100 cases of patients with senile obesity metabolic syndrome of phlegm turbid internal obstruction syndrome were randomly divided into observation group and control group,50 cases each.Control group was given conventional symptomatic treatment and dietary intervention;observation group was given Modified Wuling Powder on the basis of control group.Results The total effective rate of observation group was significantly higher than control group(P<0.05);the waist circumference,body weight,body mass index(BMI),systolic blood pressure(SBP),diastolic blood pressure(DBP),fasting blood glucose(FBG),postprandial 0.5h blood glucose(PG 0.5h),high sensitivity C-reactive protein(hs-CRP),interleukin(IL-6),tumor necrosis factor-a(TNF-a)were significantly lower than those in control group(P<0.05);there was no significant difference in PGlh,PG2h,glycosylated hemoglobin(HbAlc),total cholesterol(TC),triglyceride(TG)and low density lipoprotein cholesterol(LDL-C)between the two groups(P>0.05),and the high density lipoprotein cholesterol(HDL-C)of observation group was significantly higher than control group(P<0.05);there was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion Modified Wuling Powder has a significant effect in treatment of senile obesity metabolic syndrome of phlegm turbid internal obstruction syndrome,which can obviously improve the blood glucose,blood lipid and blood pressure levels of patients with a value of clinical application and promotion.
