中医情志护理对乳腺癌患者围手术期自我效能的影响

Effect of Emotional Nursing of Chinese Medicine on Self-Efficacy of Patients with Breast Cancer During Perioperative Period

目的:探究中医情志护理对乳腺癌患者围手术期自我效能的影响。方法:将40例乳腺癌患者,按随机数字表法将其分成2组,对照组20例采用常规护理,观察组20例采用中医情志护理。2组护理前后采用自我护理能力测定表(ESCA)评价患者自我护理能力,对比SCL-90量表评分、免疫球蛋白M (IgM),免疫球蛋白G (IgG)、免疫球蛋白A (IgA)、淋巴细胞总数(LYM)的变化情况。结果:护理后,2组各项评分较前升高,观察组护理后自我护理能力与对照组相比明显升高,差异有统计学意义(P<0.05)。2组护理后,SCL-90量表各项评分较前下降,观察组护理后偏执、恐怖、焦虑、抑郁、精神状态、强迫症、人际敏感较对照组降低,差异有统计学意义(P<0.05)。观察组护理后IgM、IgG、IgA、LYM较对照组明显升高,差异有统计学意义(P<0.05)。结论:中医情志护理对乳腺癌患者围手术期自我效能的临床影响明显,能有效提高患者术后的自我护理能力,缓解患者不良情绪,增强患者免疫系统功能。

Objective:To discuss the effect of emotional nursing of Chinese medicine on self-efficacy of patients with breast cancer during perioperative period. Methods:A total of 40 cases of patients with breast cancer were divided into two groups according to the random number table method,20 cases in each group. The control group was given routine nursing,and the observation group was given emotional nursing of Chinese medicine. Before and after nursing,self-care ability was evaluated by Exercise of Self-care Agency(ESCA) Scale,and the changes in score of SCL-90 scale,immunoglobulin M(IgM)immunoglobulin G(IgG),immunoglobulin A(IgA),and total lymphocyte count(LYM) were compared between the two groups.Results:After nursing,each score in the two groups was increased when compared with that before,and self-care ability in the observation group was significantly enhanced when compared with that in the control group, the difference being significant(P<0.05). After nursing,each score of SCL-90 scale in the two groups was decreased when compared with that before,and the scores of paranoid ideation,phobic anxiety,anxiety,depression,psychoticism,obsessive-compulsive,and interpersonal sensitivity in the observation group were decreased when compared with those in the control group,the difference being significant(P<0.05). After nursing, IgM, IgG, IgA, and LYM in the observation group were significantly increased when compared with those in the control group,the difference being significant(P<0.05). Conclusion:Emotional nursing of Chinese medicine has a significant clinical effect on the self-efficacy of patients with breast cancer during perioperative period, which can effectively enhance their postoperative self-care ability, relive adverse emotions and improve the immune system.
