直肠造瘘术后瘘口周围皮肤发炎患者应用银离子敷料的疗效观察

Efficacy observation of silver ion dressing on patients with dermatitis around the fistula after rectostomy

目的对直肠造瘘术后瘘口周围皮肤发炎患者应用银离子敷料覆盖肠造口创面的疗效进行探讨。方法66例直肠造瘘术后瘘口周围皮肤发炎患者,随机分为观察组与对照组,每组33例。对照组患者用无菌纱布及敷料覆盖创面,观察组患者应用银离子敷料覆盖创面。比较两组患者治疗前及治疗后7、14 d瘘口周围皮肤发炎程度评分,换药时视觉模拟评分法(VAS)评分、造口创面愈合时间及生活质量评分。结果治疗后7、14 d,观察组瘘口周围皮肤发炎程度评分分别为(1.36±0.15)、(0.49±0.04)分,明显低于对照组的(2.37±0.23)、(1.31±0.09)分,差异具有统计学意义(P<0.05)。观察组换药时VAS评分(2.01±0.47)分明显低于对照组的(3.96±0.71)分,造口创面愈合时间(7.01±1.47)d明显短于对照组的(11.01±3.14)d,差异具有统计学意义(P<0.05)。观察组患者生活质量评分为(88.01±7.07)分,高于对照组的(69.73±3.56)分,差异具有统计学意义(P<0.05)。结论直肠造瘘术后瘘口周围皮肤发炎患者应用银离子敷料覆盖肠造口创面,可促进创面愈合,减少炎症反应,减轻疼痛,提升患者生活质量,利于患者早日康复。

Objective To discuss the efficacy of silver ion dressing on patients with dermatitis around the fistula after rectostomy.Methods A total of 66 patient with dermatitis around the fistula after rectostomy were randomly divided into observation group and control group,with 33 cases in each group.Patients in the control group were covered with sterile gauze and dressings,while patients in the observation group were covered with silver ion dressings.The score of dermatitis around the fistula before treatment,and after 7 and 14 d of treatment,visual analogue scale(VAS)score during dressing change,wound healing time,and quality of life score were compared between the two groups.Results After 7 and 14 d of treatment,the score of dermatitis around the fistula of the observation group were(1.36±0.15)and(0.49±0.04)points,which were obviously lower than(2.37±0.23)and(1.31±0.09)points of the control group,and the difference was statistically significant(P<0.05).The VAS score during dressing change(2.01±0.47)points of the observation group was obviously lower than(3.96±0.71)points of the control group,and wound healing time(7.01±1.47)d was obviously shorter than(11.01±3.14)d of the control group.All the difference was statistically significant(P<0.05).The quality of life score of the observation group was(88.01±7.07)points,which was higher than(69.73±3.56)points of the control group,and the difference was statistically significant(P<0.05).Conclusion The application of silver ion dressing to cover the enterostomy wound in patients with dermatitis around the fistula can promote wound healing,reduce inflammation,relieve pain,improve the quality of life of the patients,and facilitate the early recovery of the patients.
