血府逐瘀汤加减治疗膝关节急性创伤性滑膜炎气滞血瘀证的效果观察

Observation on effect of modified Xuefu Zhuyu Decoction on treating acute traumatic synovitis of knee joint with Qi stagnation and blood stasis syndrome

目的分析气滞血瘀证膝关节急性创伤性滑膜炎患者的疾病治疗需求,评价中药方剂血府逐瘀汤加减辅助治疗的优势。方法84例气滞血瘀证膝关节急性创伤性滑膜炎患者,根据随机数字表法分为对照组及观察组,各42例。对照组采取滑膜炎颗粒等常规治疗,观察组在对照组基础上辅助血府逐瘀汤加减治疗。比较两组患者的治疗效果、中医证候积分、视觉模拟评分法(VAS)评分、WOMAC骨关节炎指数评分、Lysholm膝关节功能评分。结果观察组治疗总有效率90.48%高于对照组的73.81%,差异有统计学意义(P<0.05)。观察组中医证候积分(1.50±0.30)分、VAS评分(1.60±0.50)分、WOMAC骨关节炎指数评分(13.50±2.30)分均低于对照组的(4.30±1.20)、(3.20±0.70)、(18.50±3.30)分,Lysholm膝关节功能评分(84.50±5.50)分高于对照组的(75.50±4.30)分,差异有统计学意义(P<0.05)。结论血府逐瘀汤加减联合常规治疗气滞血瘀证膝关节急性创伤性滑膜炎整体效果理想,具有推广价值。

Objective To anlayze the treatment needs of patients with acute traumatic synovitis of the knee joint with Qi stagnation and blood stasis syndrome,and to evaluate the advantages of the traditional Chinese medicine Xuefu Zhuyu Decoction for adjuvant therapy.Methods A total of 84 patients with acute traumatic synovitis of the knee joint with Qi stagnation and blood stasis syndrome were divided into control group and observation group,with 42 cases in each group.The control group was treated with Huamoyan granules and other conventional treatments,and the observation group was treated with modified Xuefu Zhuyu Decoction based on the control group.The therapeutic effect,traditional Chinese medicine syndrome score,visual analogue scale(VAS)score,WOMAC osteoarthritis index score,Lysholm knee function score were compared between the two groups.Results The total effective rate of the observation group was 90.48%,which was higher than 73.81%of the control group,and the difference was statistically significant(P<0.05).The traditional Chinese medicine syndrome score(1.50±0.30)points,VAS score(1.60±0.50)points,and WOMAC osteoarthritis index score(13.50±2.30)points of the observation group were lower than(4.30±1.20),(3.20±0.70)and(18.50±3.30)points of the control group,and the Lysholm knee function score(84.50±5.50)points was higher than(75.50±4.30)points of the control group.All the difference was statistically significant(P<0.05).Conclusion Combination of modified Xuefu Zhuyu Decoction and conventional treatment has an ideal overall effect on patients with acute traumatic synovitis of the knee joint with Qi stagnation and blood stasis syndrome,and it has the value of promotion.
