DEK在乳腺癌中的表达和临床价值研究

摘要 目的探讨DEK在乳腺癌中的表达情况和临床价值。方法60例乳腺癌患者,术中进行肿瘤和癌旁组织(正常组织)病理取样,分别获得60份乳腺癌组织和60份癌旁组织样本作为研究样本。比较患者乳腺癌组织和癌旁组织DEK蛋白表达情况、免疫组化结果、DEK mRNA表达水平,统计不同肿瘤大小、病理分级和预后患者的DEK蛋白高表达情况,观察不同预后患者DEK蛋白表达情况。结果乳腺癌组织DEK蛋白相对表达量为(4.92±1.02),高于癌旁组织的(1.56±0.58),差异具有统计学意义(P<0.05)。乳腺癌组织DEK蛋白表达阳性率78.33%高于癌旁组织的8.33%,差异具有统计学意义(P<0.05)。乳腺癌组织DEK mRNA平均表达量(4.85±1.07)高于癌旁组织的(1.14±0.42),差异具有统计学意义(P<0.05)。肿瘤大小T2、T3和T4患者DEK蛋白高表达率均显著高于T1患者,差异具有统计学意义(P<0.05)。病理分级Ⅱ级和Ⅲ级患者DEK蛋白高表达率均显著高于Ⅰ级患者,差异具有统计学意义(P<0.05)。60例患者2年生存率为70.00%(42/60),死亡患者乳腺癌组织DEK蛋白高表达率94.44%(17/18)显著高于未死亡患者的71.43%(30/42),差异具有统计学意义(P<0.05)。60例患者5年生存率为60.00%(36/60),死亡患者乳腺癌组织DEK蛋白高表达率95.83%(23/24)显著高于未死亡患者的69.44%(25/36),差异具有统计学意义(P<0.05)。60例患者随访5年显示,复发或远处淋巴结转移者共28例,其乳腺癌组织DEK蛋白高表达率为89.29%(25/28)显著高于无复发和转移患者的65.63%(21/32),差异具有统计学意义(P<0.05)。结论乳腺癌患者存在肿瘤组织DEK蛋白高表达情况,而其表达水平与肿瘤大小、病理分级、预后情况存在一定联系,DEK基因具有潜在的鉴别、诊断与预后评估价值。 Objective To discuss the expression and clinical value of DEK in expression of breast cancer.Methods In 60 patients with breast cancer,pathological sampling of tumor tissue and adjacent tissue(normal tissue)were taken during the operation,and 60 breast cancer tissue and 60 adjacent tissue samples were obtained as research samples.The DEK protein expression,immunohistochemical results,and DEK mRNA expression levels in breast cancer tissues and adjacent tissues of patients were compared.The high expression of DEK protein in patients with different tumor size,pathological grade and prognosis was analyzed,and the expression of DEK protein in patients with different prognosis was observed.Results The relative expression of DEK protein in breast cancer tissue was(4.92±1.02),which was higher than that of adjacent tissues(1.56±0.58),and the difference was statistically significant(P<0.05).The positive rate of DEK protein expression in breast cancer tissue was 78.33%,which was higher than 8.33%in adjacent tissues,and the difference was statistically significant(P<0.05).The average expression of DEK mRNA in breast cancer tissue was(4.85±1.07),which was higher than(1.14±0.42)of adjacent tissues,and the difference was statistically significant(P<0.05).The high expression rate of DEK protein in patients with tumor size T2,T3 and T4 was significantly higher than that of T1 patients,and the difference was statistically significant(P<0.05).The high expression rate of DEK protein in pathological gradeⅡandⅢpatients was significantly higher than that of gradeⅠpatients,and the difference was statistically significant(P<0.05).The 2-year survival rate of 60 patients was 70.00%(42/60).The high expression rate of DEK protein in breast cancer tissues of dead patients was 94.44%(17/18),which was significantly higher than 71.43%(30/42)of non-dead patients,and the difference was statistically significant(P<0.05).The 5-year survival rate of 60 patients was 60.00%(36/60).The high expression rate of DEK protein in breast cancer tissues of dead patients was 95.83%(23/24),which was significantly higher than 69.44%(25/36)of non-dead patients,and the difference was statistically significant(P<0.05).After 5 years of follow-up,28 patients had recurrence or distant lymph node metastasis in 60 patients,of which the high expression rate of DEK protein in patients with breast cancer tissue was 89.29%(25/28),which was significantly higher than 65.63%(21/32)in patients without recurrence and metastasis,and the difference was statistically significant(P<0.05).Conclusion Breast cancer patients have high expression of DEK protein in tumor tissue,and its expression level is related to tumor size,pathological grade and prognosis.DEK gene has potential value in differentiation,diagnosis and prognosis assessment.
