前列地尔联合低分子肝素治疗高脂血症性胰腺炎临床效果的Meta分析

Meta Analysis of Clinical Efficiency of Alprostadil Combined with Low Molecular Weight Heparin in the Treatment of Hyperlipidemic Pancreatitis

目的:系统评价前列地尔联合低分子肝素治疗高脂血症性胰腺炎的临床效果。方法:检索PubMed、Eambase、Cochrane library、中国期刊全文数据库(CNKI)、万方、中国生物医学文献数据库(CBM)、维普(VIP)等数据库自建库以来至2020年5月收录的“前列地尔联合低分子肝素治疗高脂血症性胰腺炎”的相关文献,最后对纳入的文献使用Cochrane协作网提供的RevMan5.3软件合并效应量。结果:共纳入9篇文献,联合组434例,对照组429例,共计863例患者。Meta分析结果显示,联合组治疗总有效率高于对照组[RR=1.25,95%CI(1.17,-1.33),P<0.05];联合组住院时间[MD=-2.05,95%CI(-3.01,-1.09)]、症状缓解时间[MD=-1.27,95%CI(-1.54,-1.00)]、禁食时间[MD=-1.89,95%CI(-2.27,-1.51)]、淀粉酶恢复时间[MD=-1.70,95%CI(-2.28,-1.12)]均较对照组缩短(P<0.05);联合组APACHEⅡ评分[MD=-0.91,95%CI(-1.15,-0.68)]、Balthazar CT评分[MD=-1.28,95%CI(-1.41,-1.15)]、血清TG水平[MD=-1.69,95%CI(-2.07,-1.32)]均低于对照组(P<0.05)。结论:前列地尔联合低分子肝素治疗高脂血症性胰腺炎的临床效果优于单独使用前列地尔或低分子肝素。

Objective:To systematically evaluate the clinical efficiency of Alprostadil combined with Low Molecular Weight Heparin in the treatment of hyperlipidemic pancreatitis.Method:PubMed,Eambase,Cochrane Library,CNKI,Wanfang,CBM,VIP and other databases were searched for the relevant literatures on“Alprostadil combined with Low Molecular Weight Heparin in the treatment of hyperlipidemic pancreatitis”from the establishment to May 2020.Finally,Revman 5.3 software provided by Cochrane Collaboration Network was used to merge the effects.Result:A total of 9 literatures were included,including 434 cases in the combined group and 429 cases in the control group,a total of 863 cases.Meta analysis showed that the total effective rare of the combined treatment group was higher than that of the control group[RR=1.25,95%CI(1.17,-1.33),P<0.05];in the combined treatment group the hospitalization time[MD=-2.05,95%CI(-3.01,-1.09)],the duration of symptom relief[MD=-1.27,95%CI(-1.54,-1.00)],the duration of fasting[MD=-1.89,95%CI(-2.27,-1.51)],and the recovery time of serum amylase[MD=-1.70,95%CI(-2.28,-1.12)]were shorter than those of the control group(P<0.05);and the APACHEⅡscore[MD=-0.91,95%CI(-1.15,-0.68)],Balthazar CT score[MD=-1.28,95%CI(-1.41,-1.15)],serum TG level[MD=-1.69,95%CI(-2.07,-1.32)]in the combined group were lower than those in the control group(P<0.05).Conclusion:The clinical effect of Alprostadil combined with Low Molecular Weight Heparin in the treatment of hyperlipidemic pancreatitis is better than that of Alprostadil or Low Molecular Weight Heparin alone.
