2015-2019年辽宁省某医院鲍曼不动杆菌感染临床分布及耐药情况分析

Analysis of Clinical Distribution and Drug Resistance of Acinetobacter Baumannii Infection in a Hospital of Liaoning Province from 2015 to 2019

目的:分析辽宁省人民医院2015-2019年鲍曼不动杆菌分布情况及耐药率。方法:利用WHONET5.6软件对辽宁省人民医院2015-2019年分离的鲍曼不动杆菌的临床分布、标本来源、药敏结果进行分析。结果:2015-2019年共分离细菌7022株,其中鲍曼不动杆菌802株(11.4%),其5年占比分别为8.5%、11.9%、10.9%、11.6%、13.9%,2017-2019年呈逐年上升的趋势。802株鲍曼不动杆菌主要来源于痰液(88.2%)、血(2.7%)、尿(2.6%)、胆汁(1.5%)、透析液(1.1%),主要分布于重症医学科(69.1%)、呼吸内科(8.1%)、神经内科(6.0%)。本院分离的鲍曼不动杆菌2015-2019年对头孢他啶、头孢吡肟、庆大霉素、环丙沙星的耐药率均在80%以上;对多粘菌素B、替加环素的敏感性较好;对碳青霉烯类抗生素5年的耐药性均在85%以上。结论:鲍曼不动杆菌耐药情况严重,临床医生应及时了解鲍曼不动杆菌特点,规范合理使用抗生素。

Objective:To analyze the distribution and drug resistance rate of Acinetobacter baumannii in Liaoning People’s Hospital from 2015 to 2019.Method:The clinical distribution,specimen source and drug sensitivity results of Acinetobacter baumannii isolated from Liaoning People’s Hospital from 2015 to 2019 were analyzed by WHONET5.6 software.Result:A total of 7022 strains of bacteria were isolated from 2015 to 2019,among which 802 strains(11.4%)were Acinetobacter baumannii,accounting for 8.5%,11.9%,10.9%,11.6%and 13.9%,respectively,with an increasing trend year by year from 2017 to 2019.The 802 strains of Acinetobacter baumannii were mainly derived from sputum(88.2%),blood(2.7%),urine(2.6%),bile(1.5%)and dialysate(1.1%),and were mainly distributed in intensive care unit(69.1%),respiratory department(8.1%)and neurology department(6.0%).The drug resistance rates of Acinetobacter baumannii isolated in our hospital to Ceftazidime,Cefepime,Gentamicin and Ciprofloxacin from 2015 to 2019 were all more than 80%.The sensitivity to Polymyxin B and Tigecycline was good.The resistance to Carbapenems in 5 years was more than 85%.Conclusion:The drug resistance of Acinetobacter baumannii is serious,so clinicians should know the characteristics of Acinetobacter baumannii in time and standardize the rational use of antibiotics.
