不同疼痛量表在肝癌患者动脉化疗栓塞术后疼痛评估中的应用比较

Comparison of the application of different pain scales in pain assessment after trans-catheter arterial chemoembolization in patients with liver cancer

目的比较4种疼痛强度评估量表在肝癌患者肝动脉化疗栓塞(Transcatheter arterial chemoembolization,TACE)术后疼痛评估中的应用效果。方法收集106例行TACE术治疗的肝癌患者,分别采用视觉模拟评分(Visual analogue scale,VAS)、数字评定量表(Numeric rating scale,NRS)、词语描述量表(Verbal description scale,VDS)和修订版面部表情疼痛量表(Faces pain scale-revised,FPS-R)对患者进行术后疼痛强度评估,比较4种量表间的首选率、作答时间、首次应答成功率及解释次数;并分析不同性别、年龄及文化程度对患者疼痛评估的影响。结果各量表间疼痛评估结果无差异(P>0.05);4种量表之间首选率、作答时间、首次应答成功率及解释次数存在统计学差异(P<0.05),FPS-R的首选率和首次应答成功率最高,解释次数最低,VDS应答时间最短;VAS、NRS评估效果受患者年龄和文化程度的影响,VDS评估效果受患者年龄的影响,而FPS-R的评估效果在不同性别、年龄或文化程度患者中无差异(P>0.05)。结论4种疼痛评估量表均可用于TACE术后疼痛强度的评估,FPS-R临床应用效果最好。

Objective To compare the efficacy of four pain scales in pain assessment after transcatheter arterial chemoembolization(TACE)in patients with liver cancer.Methods 106 patients with liver cancer who underwent TACE were collected.Visual analogue scale(VAS),numeric rating scale(NRS),verbal description scale(VDS)and revised facial pain scale(FPS-R)were used to evaluate postoperative pain.Results There was no difference in pain assessment among the scales(P>0.05);There were statistically significant differences among the 4 scales in preferred rate,response time,first response success rate and interpretation times(P<0.05).The preferred rate and the first response success rate of FPS-R were the highest,the number of explanations was the lowest,and the response time of VDS was the shortest.VAS and NRS were affected by patients'age and education level,VDS was affected by patients'age,while FPS-R showed no difference in patients with different gender,age or education level(P>0.05).Conclusion All the four pain assessment scales can be used to evaluate the pain intensity after TACE,and the clinical application effect of FPS-R is the best.
