《艾滋病临床护理实践指南》在临床应用实践中的质性研究

Current situation of nursing practice under the application of Chinese HIV/AIDS clinical practice guidelines:A descriptive qualitative study

目的了解《艾滋病临床护理实践指南》在艾滋病病房中的应用情况以及护士应用该指南的促进因素和阻碍因素。方法2020年6-7月,选取上海市、南宁市、昆明市3所艾滋病定点医院住院病房中的一线临床护士及护理管理者为研究对象,通过半结构式访谈收集资料,采用内容分析法进行资料分析,借助Nvivo 11.0软件对访谈资料进行编码。结果共纳入16名研究对象,归纳出3个主题。分别为指南应用下护理实践的内容、护士应用指南的促进因素和阻碍因素以及指南更新的建议。结论护士应用指南的促进因素和阻碍因素包括指南相关因素、卫生系统因素以及护士个人因素。建议在更新指南时扩大关注人群,将非HIV相关慢性病管理纳入更新目录。

Objective To understand the current situation of HIV/AIDS clinical nursing practice and the application of the HIV/AIDS clinical practice guidelines in China,and the facilitators and barriers to clinical practice guideline use among nurses.Methods From June to July,2020,participants were recruited from three HIV/AIDS designated hospitals in Shanghai,Nanning city,and Kunming city.Semi-structured interviews were conducted to collect data,which were analyzed by content analysis.Data were encoded by Nvivo 11.0 software.Results 16 nurses were recruited in this study.The analysis identified three themes including the content of HIV/AIDS clinical nursing practice,facilitators and barriers to clinical practice guideline use among nurses,and suggestions for updated guidelines.Conclusion Facilitators and barriers to clinical practice guideline use among nurses included factors relating to the guideline,the individual nurse and the health care system.It was suggested that researchers should pay more attention to different population and the management of non-HIV related chronic diseases when updating the guidelines.
