基于数据挖掘李学麟教授辨治咳嗽变异性哮喘临床经验研究

Research on Clinical Experience of Professor Li Xuelin's Syndrome Differentiation in the Treatment of Cough Variant Asthma Based on Data Mining

目的基于“古今医案云平台(V2.1)”数据分析软件,分析和挖掘李学麟教授治疗咳嗽变异性哮喘(CVA)的学术思想和临床经验。方法将就诊于李学麟教授福建中医药大学附属人民医院名医苑且诊断为CVA的医案录入软件,建立数据库后导入分析池,应用软件进行医案标准化处理,运用数据挖掘分析模块进行频数分析、关联规则分析以及数据挖掘等。结果纳入的243份医案共涉及症状23个(前5种症状为咳嗽,咳痰,痰白,夜间及晨起加重,遇寒加重);共涉及中医证型13个(排名前5的证型为:表寒里饮证、风痰犯肺证、风邪犯肺证、痰湿痹阻证、风寒犯肺证);涉及中医治法10种(频次前5的治法为:解表散寒、温肺化饮;温肺化痰、理气止咳;疏风解表、化痰止咳;辛温解表、宣肺止咳;燥湿化痰、理气止咳);运用复杂系统熵聚类的药物核心组合分析算法得到的新处方为:蜜麻黄、炒苦杏仁、桂枝、白芍、细辛、干姜、五味子、茯苓、法半夏、射干、地龙、甘草。结论通过数据分析和挖掘,客观展现了李学麟教授的辨治CVA的学术思想与临床经验,为名医经验的传承与发展提供了依据和方法。

Objective:To analyze and explore Professor Li Xuelin's academic thoughts and clinical experience in the treatment of cough variant asthma(CVA)based on the“Ancient and Modern Medical Case Cloud Platform(V2.1)”data analysis software.Methods:The medical records which were diagnosed by Professor Li Xuelin as CVA patients in the famous medical garden of the People's Hospital of Fujian University of Traditional Chinese Medicine were entered into software and imported into the analysis pool after the database was established.The software was used to standardize medical records and the data mining analysis module was used for frequency analysis,association rule analysis and data mining.Results:The included 243 medical records involved a total of 23 symptoms(the first 5 symptoms were cough,sputum expectoration,white sputum,which increased at night and in the morning,and aggravated by cold).A total of 13 TCM syndromes were involved and the top 5 symptom types were exterior cold with interior fluid-retention syndrome,wind phlegm invading lung syndrome,wind-evil invading lung syndrome,phlegm-dampness block syndrome and wind-cold invading lung syndrome.10 kinds of TCM methods were involved and the first 5 treatment methods were to relieve the exterior to dispel cold,warm the lungs to change fluid-retention,warm the lungs to resolve phlegm,regulate qi to relieve cough,dispel wind to relieve the exterior and resolve phlegm to relieve cough.The new prescriptions obtained by the core combination analysis algorithm of medicines were honey ephedra,fried bitter almonds,cassia twig,paeoniae.asarum,dried ginger,schisandra,poria,pinellia,blackberry lily,earth worm and licorice.Conclusion:Through data analysis and mining,the academic thoughts and clinical experience of Professor Li Xuelin's diagnosis and treatment of CVA are objectively demonstrated,which provides a basis and method for the inheritance and development of the experience of famous doctors.
