臭氧联合黄连膏封包外敷对银屑病患者的临床疗效及其对皮损处IL-17、IL-22水平的影响

Effects of ozone combined with coptis ointment for external application on clinical symptoms,levels of IL-17 and IL-22 at skin damage sites in psoriasis patients

目的:探究臭氧联合黄连膏封包外敷对银屑病患者临床疗效及其对皮损处白细胞介素-17(IL-17)及(IL-22)水平的影响。方法:按照治疗方法不同将68例银屑病患者分成观察组(n=34)和对照组(n=34)。对照组采用消银胶囊治疗,观察组在对照组的基础上加用臭氧联合黄连膏封包外敷治疗,治疗周期均为1个月。比较治疗前、后两组患者的银屑病面积和严重程度指数评分(PASI)、瘙痒程度评分、皮损处IL-17、IL-22水平及皮肤屏障功能(角质层含水量及油脂含量),并观察治疗期间两组患者的不良反应发生情况。结果:治疗后,观察组的总有效率为91.18%,高于对照组的70.59%(P<0.05);治疗后,观察组PASI评分、瘙痒程度评分、皮损处IL-17、IL-22 mRNA水平均较治疗前降低,且均低于对照组(P<0.05);治疗后,观察组角质层含水量及油脂含量均高于对照组(P<0.05);治疗期间,两组均无明显不良反应出现。结论:与单独使用消银胶囊比较,结合臭氧黄连膏封包外敷治疗银屑病,临床疗效更显著,可降低患者皮损处IL-17、IL-22 mRNA表达水平,改善患者皮肤屏障功能。

Objective:To explore the effects of ozone combined with coptis ointment for external application on clinical symptoms,levels of interleukin-17(IL-17)and interleukin-22(IL-22)at skin damage sites in psoriasis patients.Methods:A total of 68 psoriasis patients were enrolled and divided into observation group(34 cases)and control group(34 cases)according to different treatment methods.The control group was treated with Xiaoyin Capsules,while observation group was additionally treated with ozone and coptis ointment for external application on the basis of the control group,the treatment cycle was 1 month.The clinical curative effect after treatment,scores of psoriasis area and severity index(PASI)and pruritus,levels of IL-17 and IL-22 at skin damage sites,skin barrier function(water and lipid contents in stratum corneum)before and after treatment were compared between the two groups.The occurrence of adverse reactions during treatment in both groups was observed.Results:After treatment,the total response rate of observation group was higher than that of control group(91.18%vs.70.59%,P<0.05).After treatment,the scores of PASI and pruritus,levels of IL-17 and IL-22mRNA at skin damage sites in observation group were decreased,the observation group was lower than the control group(P<0.05).After treatment,water and lipid contents in stratum corneum of observation group were higher than those of control group(P<0.05).During treatment,there were no significant adverse reactions in either group.Conclusion:Compared with Xiaoyin Capsule alone,the curative effect of ozone combined with coptis ointment for external application is significant on psoriasis.It can effectively improve skin lesions and pruritus symptoms of patients,decrease the expression levels of IL-17 and IL-22mRNA at skin damage sites,and improve skin barrier function.
