中药扶正解毒汤联合三七化瘀口服液对脓毒血症患者中医证候积分的影响

Effect of Fuzheng Jiedu Decoction Combined with Sanqi Huayu Oral Liquid on TCM Syndrome Scores in Patients with Sepsis

目的探讨中药扶正解毒汤联合三七化瘀口服液治疗脓毒血症的临床疗效,以及对患者中医证候积分的影响。方法选取医院2018年2月至2020年2月收治的脓毒血症患者88例,按信封法随机分为观察组和对照组,各44例。两组患者均予脓毒血症的常规治疗,并予三七化瘀口服液治疗,观察组患者加用中药扶正解毒汤,均连续治疗2周。结果观察组总有效率为90.91%,显著高于对照组的72.73%(P<0.05);观察组患者的中医证候(高热、神昏、气促、腹胀)评分均显著低于对照组(P<0.05);观察组患者的血流动力学指标(心指数、平均动脉压)均显著高于对照组,外周血管阻力指数显著低于对照组(P<0.05);观察组患者的心肌损伤指标(血清降钙素原、血乳酸、脑钠肽)水平均显著低于对照组(P<0.05);观察组和对照组不良反应发生率相当(15.91%比9.09%,P>0.05)。结论中药扶正解毒汤联合三七化瘀口服液治疗脓毒血症疗效较好,能降低患者的中医证候积分,改善血流动力学指标水平和心肌损伤程度,且治疗安全性良好。

Objective To investigate the clinical efficacy of Fuzheng Jiedu Decoction combined with Sanqi Huayu Oral Liquid in the treatment of patients with sepsis and its effect on TCM syndrome scores.Methods A total of 88 patients with sepsis admitted to our hospital from February 2018 to February 2020 were selected and randomly divided into the observation group and the control group according to the envelope blind sampling method,44 cases in each group.The patients in the two groups were given the routine treatment of sepsis and Sanqi Huayu Oral Liquid,on this basis,the patients in the observation group were treated with Fuzheng Jiedu Decoction.Both groups were continuously treated for two weeks.Results The total effective rate of the observation group was 90.91%,which was significantly higher than 72.73%of the control group(P<0.05).After treatment,t he TCM syndrome scores(high fever,dizziness,shortness of breath,abdominal distension)in the observation group were significantly lower than those in the control group(P<0.05).The hemodynamic indexes(cardiac index and mean arterial pressure)in the observation group were significantly higher than those in the control group,while the peripheral vascular resistance index in the observation group was significantly lower than that in the control group(P<0.05).The myocardial injury indexes[serum procalcitonin(PCT),lactate(LAC)and brain natriuretic peptide(BNP)]in the observation group were significantly lower than those in the control group(P<0.05).The incidence of adverse reactions in the observation group was similar to that in the control group(15.91%vs.9.09%,P>0.05).Conclusion Fuzheng Jiedu Decoction combined with Sanqi Huayu Oral Liquid is effective and safe in the treatment of sepsis,which can reduce the TCM syndrome scores,and improve the levels of hemodynamics and myocardial injury.
