阿卡波糖与二甲双胍联合预混胰岛素治疗2型糖尿病的效果比较

Comparison of the effects of acarbose and metformin combined with premixed insulin in the treatment of type 2 diabetes

目的比较阿卡波糖与二甲双胍联合预混胰岛素治疗2型糖尿病(T2DM)的临床效果。方法选取2019年6月-2020年6月华中科技大学医院诊治的T2DM患者124例开展本次试验,采用随机数字表法将所有患者均分为观察组和对照组,各62例。对照组给予二甲双胍联合预混胰岛素治疗,观察组给予阿卡波糖联合预混胰岛素治疗。比较2组患者治疗前后血糖水平,治疗总有效率,不良反应发生率及患者治疗前后治疗依从性。结果治疗3个月后,2组糖化血红蛋白(HbA1c)、空腹血糖(FPG)水平均较治疗前下降(P<0.01),且观察组降低幅度大于对照组(P均<0.01);治疗3个月,观察组总有效率为98.39%,高于对照组的87.10%(x^(2)=5.871,P=0.032);治疗3个月后,2组患者Morisky用药依从性量表(MMAS)评分均较治疗前升高(P<0.01),且观察组MMAS评分高于对照组(P<0.01);用药期间,观察组不良反应总发生率为3.22%,低于对照组的12.90%(x^(2)=3.916,P=0.048)。结论T2DM患者给予阿卡波糖联合预混胰岛素治疗可取得显著效果,能够明显改善患者血糖水平,提高治疗总有效率并降低不良反应发生率,且患者治疗依从性高,具有推广应用价值。

Objective To compare the clinical effects of acarbose and metformin combined with premixed insulin in the treatment of type 2 diabetes(T2DM).Methods 124 patients with T2DM patients in Huazhong University of Science and Technology Hospital from June 2019 to June 2020 were selected to carry out the experimental study.All patients were randomly divided into control group and observation group,with 62 cases in each group.The control group was treated with metformin combined with premixed insulin,and the observation group was treated with acarbose combined with premixed insulin.The blood glucose levels before and after treatment,the total effective rate of treatment,the incidence of adverse reactions and the treatment compliance of the patients before and after treatment were compared between the two groups.Results After 3 months of treatment,the levels of glycosylated hemoglobin(HbA1c)and fasting blood glucose(FPG)in the two groups were lower than before treatment(P<0.01),and the reduction in the observation group was greater than that of the control group(P<0.01).After 3 months of treatment,the total effective rate in the observation group was 98.39%,which was higher than 87.10%in the control group(x^(2)=5.871,P=0.032).During the medication period,the total incidence of adverse reactions in the observation group was 3.22%,which was lower than 12.90%in the control group(x^(2)=3.916,P=0.048).After 3 months of treatment,the Morisky Medication Compliance Scale(MMAS)scores of the two groups were higher than before treatment(P<0.01),and the MMAS score of the observation group was higher than that of the control group(P<0.01).Conclusion Acarbose treatment in T2DM patients can achieve more significant clinical efficacy,can significantly improve the patients blood glucose level,increase the total effective rate of treatment and reduce the incidence of adverse reactions,and the patients have high treatment compliance,which has the value of promotion and application.
