桃红四物汤合防己黄芪汤治疗胫腓骨骨折术后肿胀的临床效果

Clinical Effects of Taohong Siwu Decoction Combined with Fangji Huangqi Decoction on Postoperative Swelling of Tibia and Fibula Fractures

目的:研究桃红四物汤合防己黄芪汤治疗胫腓骨骨折术后肿胀的临床效果。方法:将2019年6月-2020年12来笔者所在医院治疗的70例胫腓骨骨折术后肿胀患者纳入研究,并按照随机数字表法分为A组(35例)和B组(35例)。A组患者给予常规治疗,B组患者在A组基础上给予桃红四物汤合防己黄芪汤,两组患者均连续治疗7 d。比较两组患者治疗后临床疗效、治疗前及治疗后3、7 d的肿胀程度,以及治疗前后血清骨形态发生蛋白-2(BMP-2)、血管内皮生长因子(VEGF)、肿瘤坏死因子-α(TNF-α)水平。结果:治疗后B组的总有效率为94.29%,高于A组的74.29%(P<0.05), 与治疗前相比,治疗后3、7 d两组患者肿胀程度均减小,且B组小于A组(P<0.05), 与治疗前比,治疗后两组患者血清BMP-2、VEGF水平均升高,且B组高于A组,而血清TNF-α水平均下降,且B组低于A组(P<0.05)。结论:桃红四物汤合防己黄芪汤能够有效消除胫腓骨骨折术后肿胀患者的肿胀程度,同时抑制炎症反应,加速骨折愈合,疗效显著。

Objective:To investigate the clinical effects of Taohong Siwu Decoction combined with Fangji Huangqi Decoction on postoperative swelling of tibia and fibula fractures.Method:A total of 70 patients with postoperative swelling of tibia and fibula fracture treated in our hospital from June 2019 to December 2020 were selected and divided into the group A(35 cases)and the group B(35 cases)according to the random number table method.Patients in the group A were given conventional treatment,and patients in the group B were given Taohong Siwu Decoction combined with Fangji Huangqi Decoction on the basis of the group A,and patients in both groups were treated continuously for 7 d.The clinical outcomes,swelling before and 3 and 7 d after treatment,and serum levels of serum bone morphogenetic protein-2(BMP-2),vascular endothelial growth factor(VEGF),tumor necrosis factor-α(TNF-α)before and after treatment were compared between the two groups.Result:The overall response rate of the group B patients was 94.29%after treatment,which was higher than the rate of 74.29%in the group A(P<0.05).Compared with before treatment,the degrees of swelling of 3 d,7 d decreased gradually in both groups and which were lower in the group B than those in the group A(P<0.05).Compared with before treatment,the serum levels of BMP-2 and VEGF increased in both groups after treatment,and the indexes in the group B was higher than those in the group A,whereas the serum levels of TNF-αdecreased in both groups,and the group B was lower than the group A(P<0.05).Conclusion:The combination of Taohong Siwu Decoction combined with Fangji Huangqi Decoction can effectively eliminate the degree of swelling in patients with postoperative swelling due to tibia and fibula fracture,at the same time,inhibit the inflammatory response and accelerate fracture healing,with significant curative effect.
