产前超声征象评分对前置胎盘伴胎盘植入的诊断价值及准确性分析

Analysis of the Diagnostic Value and Accuracy of Prenatal Ultrasound Sign Score for Placenta Previa with Placenta Accreta

目的:探讨产前超声征象评分诊断前置胎盘伴胎盘植入的价值。方法:将笔者所在医院2018年2月-2019年11月收治40例前置胎盘伴胎盘植入患者设为A组,将同期收治的40例前置胎盘未伴胎盘植入患者设为B组,将40例未伴前置胎盘及胎盘植入孕妇设为C组。所有患者均进行超声征象评分。比较三组产前超声征象评分,比较不同类型胎盘植入患者产前超声征象评分。分析产前超声征象评分诊断前置胎盘伴胎盘植入的价值。结果:A组产前超声征象评分高于B、C组,差异有统计学意义(P<0.05), 植入型、穿透型患者产前超声征象评分高于粘连型患者,差异有统计学意义(P<0.05), 产前超声征象评分诊断前置胎盘伴胎盘植入的灵敏度为92.50%(37/40),诊断特异度为97.50%(78/80),诊断准确度为95.83%(115/120)。结论:产前超声征象评分可用于诊断前置胎盘伴胎盘植入,诊断准确度较高,具有显著的诊断价值。

Objective:To explore the value of prenatal ultrasound sign score in the diagnosis of placenta previa with placenta accreta.Method:A total of 40 patients with placenta previa and placenta accreta admitted in our hospital from February 2018 to November 2019 were set as group A,40 patients with placenta previa without placenta accreta admitted in the same period were set as group B,and 40 pregnant women without placenta previa and placenta accreta were set as group C.All patients were scored with ultrasound sign score.Prenatal ultrasound sign scores of the three groups were compared,and the prenatal ultrasound sign score of patients with different types of placenta accreta were compared.The value of prenatal ultrasound sign score in the diagnosis of placenta previa and placenta accreta was analyzed.Result:The prenatal ultrasound sign score of group A was higher than that of group B and C,and the difference was statistically significant(P<0.05).The prenatal ultrasound sign score of patients with implant type and penetration type were higher than that of patients with adhesion type,and the difference was statistically significant(P<0.05).The sensitivity,specificity and accuracy of prenatal ultrasound sign score in the diagnosis of placenta previa and placenta accreta were 92.50%(37/40),97.50%(78/80)and 95.83%(115/120).Conclusion:Prenatal ultrasound sign score can be used in the diagnosis of placenta previa and placenta accreta,with high diagnostic accuracy and significant diagnostic value.
