新生儿高胆红素血症严重程度与肾功能指标、心肌酶谱的相关性分析 被引量:3

摘要 目的:探究不同严重程度高胆红素血症新生儿肾功能指标和心肌酶谱指标的临床表达。方法:选取本院2019年1月-2020年1月收治的110例足月高胆红素血症新生儿设为试验组,根据总胆红素水平(TBiL)将患儿分为轻度组(n=59)和中重度组(n=51),另选取同期体检结果健康的新生儿46例设为对照组。检测两组研究对象肾功能、心肌酶谱各指标,并进行Pearson相关性分析。结果:不同程度高胆红素血症新生儿与对照组一般资料比较,差异无统计学意义(P>0.05),轻度组、中重度组TBiL水平明显高于对照组(P<0.05),三组尿素氮(BUN)、尿酸(UA)水平比较,差异无统计学意义(P>0.05),轻度组和中重度组血肌酐(Scr)水平均明显低于对照组(P<0.05),轻度组和中重度组肌酸激酶(CK)、α-羟丁酸脱氢酶(HBDH)和乳酸脱氢酶(LDH)水平明显高于对照组(P<0.05),且中重度组CK、HBDH和LDH水平明显高于轻度组(P<0.05)。高胆红素血症患儿血清CK、HBDH和LDH水平与严重程度呈正相关(r=0.783、0.694、0.593,P=0.000),高胆红素血症患儿血清Scr与严重程度呈负相关关系(r=-0.611,P=0.000)。结论:血清Scr、CK、HBDH、LDH水平可有效反映新生儿高胆红素血症严重程度,可为临床诊治提供依据。 Objective:To explore the clinical expression of renal function and myocardial enzymes in neonates with hyperbilirubinemia of different severity.Method:A total of 110 cases of full-term hyperbilirubinemia newborns in our hospital from January 2019 to January 2020 were selected as the experimental group,and they were divided into the mild group(n=59)and the moderate severe group(n=51)according to the total bilirubin level(TBiL),and 46 healthy newborns in the same period were selected as the control group.The indexes of renal function and myocardial enzymes were detected,and Pearson correlation analysis was performed.Result:Compared with the general data of the control group,there were no significant differences in different degrees of hyperbilirubinemia(P>0.05).The TBiL levels of the mild group and the moderate and severe group were significantly higher than those of the control group(P<0.05).There were no significant differences in the levels of urea nitrogen(BUN)and uric acid(UA)among the three groups(P>0.05).The serum creatinine(Scr)levels of the mild group and the moderate and severe group were significantly lower than those in the control group(P<0.05),the levels of creatine kinase(CK),α-hydroxybutyrate dehydrogenase(HBDH)and lactate dehydrogenase(LDH)were significantly higher than those in the control group(P<0.05),the levels of CK,HBDH and LDH in moderate severe group were significantly higher than those in mild group(P<0.05).The serum levels of CK,HBDH and LDH were positively correlated with the severity of hyperbilirubinemia(r=0.783,0.694,0.593,P=0.000);the serum levels of Scr were negatively correlated with the severity of hyperbilirubinemia(r=-0.611,P=0.000).Conclusion:The serum levels of Scr,CK,HBDH and LDH can effectively reflect the severity of neonatal hyperbilirubinemia,which can provide the basis for clinical diagnosis and treatment.
