艾滋病相关淋巴瘤患者不良预后发展的危险因素

Risk Factors for the Development of Poor Prognosis in Patients with AIDS Related Lymphoma

目的:探讨艾滋病(AIDS)相关淋巴瘤患者不良预后发展的危险因素,旨在为临床治疗提供指导性意见。方法:选取2017年1月-2020年1月于本院接受治疗的62例AIDS相关淋巴瘤患者,均予以AIDS相关淋巴瘤常规治疗并随访12个月。依据预后情况分为预后良好组与预后不良组,统计患者性别、年龄、AIDS传播途径、文化程度、淋巴瘤临床分期、病灶数量、接受化学治疗、接受HAART治疗一般资料,分析导致AIDS相关淋巴瘤患者预后不良的影响因素。结果:经12个月随访后,62例AIDS相关淋巴瘤患者预后良好29例,预后良好率46.77%,预后不良33例,预后不良率53.23%, 经单因素分析显示,AIDS相关淋巴瘤患者预后不良不受性别、AIDS传播途径、淋巴瘤临床分期、病灶数量的影响(P>0.05), 可能受年龄、文化程度、接受化学治疗与HAART治疗的影响(P=0.000), 经Logistic回归分析得出,文化程度偏低、未接受化学治疗与HAART治疗是导致AIDS相关淋巴瘤患者预后不良的影响因素(OR>1,P=0.000)。结论:文化程度偏低、未接受化学治疗与HAART治疗AIDS相关淋巴瘤患者易发生预后不良,临床可据此予以相应干预措施以改善患者预后。

Objective:To explore the risk factors of poor prognosis in AIDS related lymphoma patients,and to provide guidance for clinical treatment.Method:A total of 62 patients with AIDS related lymphoma who were treated in our hospital from January 2017 to January 2020 were selected.They received routine treatment for AIDS related lymphoma and were followed up for 12 months.According to the prognosis,they were divided into good prognosis group and poor prognosis group.Gender,age,route of transmission of AIDS,educational level,clinical stage of lymphoma,number of lesions,chemotherapy and HAART treatment were analyzed to analyze the influencing factors of poor prognosis of AIDS related lymphoma patients.Result:After 12 months of follow-up,the prognosis of 62 AIDS related lymphoma patients was good in 29 cases(46.77%),and poor in 33 cases(53.23%).Univariate analysis showed that the poor prognosis of AIDS related lymphoma patients was not affected by gender,route of transmission of AIDS,clinical stage of lymphoma and the number of lesions(P>0.05);it might be affected by age,education level and chemotherapy and HAART treatment(P=0.000).Logistic regression analysis showed that low education level,no chemotherapy and HAART treatment were the influencing factors of poor prognosis of AIDS related lymphoma patients(OR>1,P=0.000).Conclusion:Patients with low education level,no chemotherapy and HAART treatment of AIDS related lymphoma are prone to have poor prognosis,so corresponding intervention measures can be taken to improve the prognosis of patients。
