经皮肝动脉置管介入热化疗治疗不能手术切除的结直肠癌肝转移瘤的临床效果

Clinical Effect of Percutaneous Hepatic Artery Catheterization Interventional Thermo Chemotherapy in the Treatment of Unresectable Liver Metastases from Colorectal Cancer

目的:探讨经皮肝动脉置管介入热化疗治疗不能手术切除的结直肠癌肝转移瘤的临床效果。方法:选取2016年10月-2018年6月本院收治的不能手术切除的结直肠癌肝转移瘤患者46例,应用随机数字法分为两组,对照组应用全身静脉姑息化疗治疗,研究组应用经皮肝动脉置管介入热化疗治疗。观察分析两组的临床疗效、1年生存率、不良反应、肝功能、Karnofsky评分及生存期情况。结果:与对照组比,研究组临床疗效及1年生存率较高,不良反应发生率较低(P<0.05);与对照组比,研究组AST、ALT及TBIL水平上升不明显,ALB水平无明显下降(P>0.05);与对照组比,研究组Karnofsky评分较高,生存期较长(P<0.05)。结论:经皮肝动脉置管介入热化疗治疗不能手术切除结直肠癌肝转移瘤患者效果显著,不仅可以减轻对患者肝功能的损害,减少不良反应,提升其生存率,还可以提高患者对化疗的承受能力,具有较高的安全性,值得临床大力推广。

Objective:To investigate the clinical effect of percutaneous hepatic artery catheterization interventional thermo chemotherapy in the treatment of unresectable liver metastases from colorectal cancer.Method:A total of 46 patients with unresectable liver metastases from colorectal cancer admitted to our hospital from October 2016 to June 2018 were selected and divided into two groups by random number method.The control group was treated with systemic venous palliative chemotherapy,and the study group was treated with percutaneous hepatic artery interventional thermo chemotherapy.Clinical efficacy,1-year survival rate,adverse reactions,liver function,Karnofsky score and survival time of the two groups were observed and analyzed.Result:Compared with the control group,the clinical efficacy and 1-year survival rate of the study group were higher,and the incidence of adverse reactions was lower(P<0.05).Compared with the control group,the levels of AST,ALT and TBIL in the study group were not significantly increased,and the level of ALB was not significantly decreased(P>0.05).Compared with the control group,the study group had higher Karnofsky score and longer survival(P<0.05).Conclusion:Percutaneous hepatic arterial interventional thermo chemotherapy has a significant effect on patients with colorectal cancer liver metastases that cannot be surgically resected.It can not only reduce the damage to the liver function of patients,reduce adverse reactions,improve their survival rate,but also improve the patients’tolerance to chemotherapy,with high safety,and is worthy of vigorous clinical promotion.
