电针联合易罐疗法对腰椎间盘突出症坐骨神经痛患者复发、疼痛情况及行走功能的影响

Influence of Electroacupuncture Combined with Yiguan Therapy on Recurrence,Pain and Walking Function of Lumbar Disc Herniation Patients with Sciatica

目的:探讨电针联合易罐疗法对腰椎间盘突出症坐骨神经痛患者复发、疼痛情况及行走功能的影响。方法:选择2019年1月-2020年5月在本院治疗的腰椎间盘突出症坐骨神经痛患者90例为研究对象,按随机数字表法分为对照组和研究组,各45例。对照组给予温针治疗,研究组实施电针联合易罐治疗,均治疗2周。两组随访时间6个月,记录其复发情况;对比两组治疗前及治疗2周后的视觉模拟评分(VAS)、疼痛分级指数(PRI)、现时疼痛强度(PPI)、Barthel指数评分;对比两组直腿抬高实验正常时间、行走功能改善时间。结果:随访6个月,研究组复发率(4.44%)明显低于对照组(20.00%),差异有统计学意义(χ^(2)=5.075,P=0.014)。治疗2周后两组VAS、PRI、PPI评分均较治疗前降低,研究组低于对照组,差异有统计学意义(P<0.05)。治疗前两组Barthel指数评分比较差异无统计学意义(P>0.05);治疗2周后研究组Barthel指数评分明显高于对照组,差异有统计学意义(P<0.05)。研究组直腿抬高实验正常时间、行走功能改善的时间均早于对照组,差异有统计学意义(P<0.05)。结论:电针联合易罐疗法可显著缓解腰椎间盘突出症坐骨神经痛的疼痛情况,降低其复发率,改善生活自理能力,在临床上可广泛使用。

Objective:To explore the influence of electroacupuncture combined with Yiguan therapy on recurrence,pain and walking function of lumbar disc herniation patients with sciatica.Method:Ninety patients with sciatica with lumbar disc herniation treated in our hospital from January 2019 to May 2020 were selected as the study objects and randomly divided into the control group and the study group,45 cases in each group.The control group was treated with warm acupuncture,the study group was treated with electroacupuncture combined with Yiguan,all patients were treated for 2 weeks.The patients in the two groups were followed up for 6 months,and the recurrence was recorded;the visual analogue scale (VAS),pain rating index (PRI),present pain intensity (PPI),Barthel index score before and after 2 weeks of treatment were compared between the two groups;the normal time of straight leg raising test and the improvement time of walking function were compared between the two groups.Result:After 6 months follow-up,the recurrence rate of the study group (4.44%) was significantly lower than 20.00% of the control group,and the difference was statistically significant (χ^(2)=5.075,P=0.014).After 2 weeks of treatment,the VAS,PRI,PPI scores of the two groups were decreased than those before treatment,and those of the study group were lower than those of the control group,the differences were statistically significant (P<0.05).There was no significant difference in Barthel index score between the two groups before treatment (P>0.05);after 2 weeks of treatment,the Barthel index score of the study group was significantly higher than that of the control group,and the difference was statistically significant (P<0.05).The normal time of straight leg raising test and the improvement time of walking function in the study group were earlier than those in the control group,the differences were statistically significant (P<0.05).Conclusion:Electroacupuncture combined with Yiguan therapy can significantly relieve the pain of sciatica of patients with lumbar disc herniation,reduce its recurrence rate,and improve self-care ability.It can be widely used in clinic.
