STARR术联合消痔灵注射治疗直肠黏膜内脱垂的临床效果

Clinical Effect of STARR Combined with Xiaozhiling Injection in the Treatment of Rectal Mucosal Prolapse

目的:探究在直肠黏膜内脱垂患者的治疗中联合应用经肛吻合器直肠切除术(STARR)和消痔灵注射液的临床效果。方法:选取2017年1月-2020年7月本院治疗的66例直肠黏膜内脱垂患者,通过随机方式按照1∶1的比例分为对照组和研究组,各33例。给予对照组STARR手术治疗,研究组联合应用消痔灵注射液。比较两组治疗效果、症状积分、并发症发生率。结果:两组治疗总有效率分别为96.97%(32/33)和78.79%(26/33),研究组治疗效果显著优于对照组(χ^(2)=5.12,P=0.0236);治疗前两组便后肛门坠胀、排便费力、肛门阻塞的症状评分比较差异无统计学意义(P>0.05),治疗后研究组各项症状积分均明显低于对照组(t=8.111、7.056、7.701,P=0.000);两组肿胀出血、排尿困难、肛周疼痛和感染的总发生率分别为6.06%(2/33)和27.27%(9/33),研究组并发症发生率明显低于对照组(χ^(2)==5.35,P=0.0208)。结论:联合应用STARR术和消痔灵注射液治疗直肠黏膜内脱垂,可以有效促进患者临床症状的改善,减少术后并发症发生,提高临床疗效。

Objective:To explore the clinical effect of transanal stapler rectal resection (STARR) and Xiaozhiling Injection in the treatment of rectal mucosal prolapse.Method:A total of 66 rectal mucosal prolapse patients treated in our hospital from January 2017 to July 2020 were selected and the patients were randomly divided into the control group and the study group according to the ratio of 1∶1,with 33 cases in each group.The control group was treated with STARR operation,and the study group was combined with Xiaozhiling Injection.The treatment effect,symptom score and incidence of complications between the two groups were compared.Result:The total effective rates of the two groups were 96.97% (32/33) and 78.79% (26/33),respectively,and the treatment effect of the study group was significantly better than that of the control group (χ^(2)=5.12,P=0.023 6);before treatment,there were no significant differences in the symptom scores of anal distension,defecation difficulty and anal obstruction between the two groups (P>0.05),after treatment,the symptom scores of the study group were significantly lower than those of the control group (t=8.111,7.056,7.701,P=0.000).The total incidence of swelling and bleeding,dysuria,perianal pain and infection in the two groups were 6.06% (2/33) and 27.27% (9/33),respectively,the incidence of complications in the study group was significantly lower than that in the control group (χ^(2)=5.35,P=0.020 8).Conclusion:Combined application of STARR and Xiaozhiling Injection in the treatment of rectal mucosal prolapse can effectively improve the clinical symptoms of patients,reduce the incidence of postoperative complications,and improve the clinical efficacy.
