精心护理理念在植入式静脉输液港化疗乳腺癌患者中的应用

Application of Meticulous Nursing Concept in Implantable Intravenous Port Chemotherapy for Breast Cancer Patients

目的:探讨精心护理理念对植入式静脉输液港化疗乳腺癌患者依从性及并发症的影响。方法:选择本院2018年1月-2020年12月收治的乳腺癌患者71例为研究对象,根据随机数字表法分为观察组(n=39)和对照组(n=32),两组均采用植入式静脉输液港,观察组予以精心护理理念护理,对照组予以常规护理。对比两组护理满意度、依从性、生命质量评分及并发症与不良事件。结果:护理后,观察组护理总满意度高于对照组(89.74%vs 68.75%),差异有统计学意义(P<0.05);观察组导管维护费用、导管维护时间、再次置管率及意外拔管率均优于对照组[(155.83±19.28)元vs(358.34±42.14)元、(7.25±0.84)min vs(11.48±2.23)min、0 vs 15.63%、0 vs 12.50%],差异有统计学意义(P<0.05);观察组心理功能、躯体功能、社会功能及日常活动评分均高于对照组(P<0.05);观察组并发症及不良事件发生率低于对照组(7.69%vs 25.00%),差异有统计学意义(P<0.05)。结论:精心护理理念能有效提高植入式静脉输液港化疗乳腺癌患者的临床护理效果及依从性,提高患者生命质量,降低患者并发症及不良事件发生率,值得临床推广应用。

Objective:To explore the influence of meticulous nursing concept on the compliance and complications of breast cancer patients undergoing chemotherapy in implantable intravenous port.Method:Seventy-one cases of breast cancer patients admitted to our hospital from January 2018 to December 2020 were selected as the research objects,and were divided into the observation group(n=39)and the control group(n=32)according to random number table method.Patients in the two groups were treated with implantable intravenous infusion port,the observation group was given meticulous nursing concept nursing,and the control group was given routine nursing.The nursing satisfaction,compliance,quality of life score and complications were compared between the two groups.Result:After nursing,the total nursing satisfaction of the observation group was higher than that of the control group(89.74%vs 68.75%),the difference was statistically significant(P<0.05);the catheter maintenance cost,catheter maintenance time,catheter replacement rate and accidental extubation rate in the observation group were better than those in the control group[(155.83±19.28)yuan vs(358.34±42.14)yuan,(7.25±0.84)min vs(11.48±2.23)min,0 vs 15.63%,0 vs 12.50%],the differences were statistically significant(P<0.05);the scores of mental function,physical function,social function and daily activity in the observation group were higher than those in the control group(P<0.05).The incidence of complications,adverse events in the observation group was lower than that in the control group(7.69%vs 25.00%),the difference was statistically significant(P<0.05).Conclusion:Meticulous nursing concept can effectively improve the clinical nursing effect and compliance of implantable intravenous port chemotherapy for breast cancer patients,improve the quality of life of patients,reduce the incidence of complications and adverse events,and is worthy of clinical promotion and application.
