椎间孔镜下髓核摘除联合神经根阻滞治疗脱垂型腰椎间盘突出症临床效果及并发症分析

Clinical Effect and Complications Analysis of Foraminal Nucleus Pulposus Removal Combined with Nerve Root Block in the Treatment of Prolapsed Lumbar Disc Herniation

目的:分析椎间孔镜下髓核摘除联合神经根阻滞治疗脱垂型腰椎间盘突出症临床效果及并发症情况。方法:选择2019年1月-2020年7月本院收治脱垂型腰椎间盘突出症94例患者,按随机数表分为两组,对照组47例给予椎间孔镜下髓核摘除手术,研究组47例实施椎间孔镜下髓核摘除联合神经根阻滞治疗,对两组临床效果及并发症进行比较。结果:研究组术后1 d VAS评分(3.41±1.17)分,术后1个月(2.40±0.83)分,术后6个月(0.87±0.26)分,均低于对照组(P<0.05);研究组术后1 d的ODI(39.37±13.31)%,术后1个月(23.14±7.38)%,术后6个月(10.21±2.12)%,均显著优于对照组(P<0.05);研究组疼痛、麻木、复发等并发症发生率6.38%,比对照组的21.27%低(P<0.05)。结论:椎间孔镜下髓核摘除联合神经根阻滞治疗脱垂型腰椎间盘突出症患者,可改善功能障碍,减轻疼痛程度,能减少术后并发症发生。

Objective:To analyze the clinical effects and complications of foraminal nucleus pulposus removal combined with nerve root block in the treatment of prolapsed lumbar disc herniation.Method:From January 2019 to July 2020,94 patients with prolapsed lumbar intervertebral disc herniation were selected and divided into two groups according to a random number table.For the control group,47 patients were treated with foraminal nucleus pulposus surgery.Forty-seven patients in the study group underwent foraminal removal of nucleus pulposus combined with nerve root block therapy,and the clinical effects and complications of the two groups were compared.Result:The VAS scores of the study group at 1 d(3.41±1.17)points,1 month(2.40±0.83)points and 6 months(0.87±0.26)points after operation,were lower than those of the control group(P<0.05).And the ODI of study group at 1 d after operation(39.37±13.31)%,1 month after operation(23.14±7.38)%and 6 months after operation(10.21±2.12)%,were better than those in the control group(P<0.05).The rate of complications such as pain,numbness,recurrence in the study group was 6.38%,which was lower than 21.27%of the control group(P<0.05).Conclusion:Foraminal nucleus pulposus removal combined with nerve root block to treat patients with prolapsed lumbar disc herniation can improve dysfunction,reduce pain,and reduce postoperative complications.
