急性脑出血短期预后不良患者血清白细胞介素-37及淀粉样蛋白A的变化及临床意义 被引量:3

Changes and clinical significance of serum interleukin-37 and serum amyloid A in patients with poor short-term prognosis after acute cerebral hemorrhage

摘要 目的探讨急性脑出血患者血清白细胞介素-37(interleukin-37,IL-37)及淀粉样蛋白A(serum amyloid A,SAA)的变化及临床意义。方法收集本院2018年1月—2019年12月收治的急性脑出血患者168例,依据mRS评分结果将患者分为预后良好组和预后不良组。分析影响急性脑出血患者短期预后不良的危险因素,评价IL-37和SAA对急性脑出血患者短期预后不良的预测价值。结果预后不良组患者NIHSS评分、IL-37和SAA水平均明显高于预后良好组患者,组间两两比较,差异均有统计学意义(t值分别为6.251、12.366、18.169,P<0.001)。IL-37和SAA对急性脑出血患者预后不良均有较好的预测价值,且相比于IL-37、SAA的预测价值更高。出血量、破入脑室、NIHSS评分、IL-37和SAA是影响急性脑出血患者短期预后不良的危险因素(P<0.05)。结论IL-37和SAA对急性脑出血患者短期内预后不良具有较好的预测价值,且相比于IL-37,SAA的预测价值更为显著。 Objective To explore the changes and clinical significance of serum interleukin-37 and serum amyloid A in patients with acute cerebral hemorrhage.Methods A total of 168 patients with acute cerebral hemorrhage admitted to our hospital from January 2018 to January 2019 were collected.According to the mRS score results,patients were divided into good prognosis group and poor prognosis group.The risk factors affecting short-term prognosis in patients with acute cerebral hemorrhage were conducted for analysis,and the predictive value of IL-37 and SAA in the short-term prognosis of patients with acute cerebral hemorrhage was also evaluated.Results The NIHSS score,IL-37 and SAA levels of patients with poor prognosis were higher than those with good prognosis,with the differences statistically significant(t=6.251,t=12.366,t=18.169,P<0.001).IL-37 and SAA have good predictive value for patients with poor prognosis of acute cerebral hemorrhage.And compared with IL-37,the predicted value of SAA is higher.Bleeding volume,rupture into the ventricle,NIHSS score,IL-37 and SAA are risk factors that affect the short-term prognosis of patients with acute cerebral hemorrhage(P<0.05).Conclusion IL-37 and SAA have good predictive value for short-term poor prognosis in patients with acute cerebral hemorrhage.And compared with IL-37,the predictive value of SAA is more significant.
