载脂蛋白B与载脂蛋白A-I的比值与多囊卵巢综合征患者痤疮皮损的相关性研究

摘要 目的探讨载脂蛋白B(ApoB)与载脂蛋白A-I(ApoA-I)的比值(ApoB/ApoA-I)对多囊卵巢综合征(PCOS)患者痤疮程度的影响。方法选取2018年6月至8月就诊于自贡市妇幼保健院的115例PCOS患者作为研究对象。根据痤疮综合评分系统(GAGS)对患者的皮损程度进行评分;比较不同痤疮程度患者血脂、性激素、胰岛素抵抗、ApoB/ApoA-I差异;分析ApoB/ApoA-I和GAGS评分与患者代谢和内分泌临床参数的相关性。结果 PCOS患者痤疮程度与胰岛素抵抗指数(HOMA-IR)、体重指数(BMI)、睾酮(T)、ApoB/ApoA-I以及血脂水平相关;去除胰岛素抵抗、肥胖和T的贡献后,ApoB/ApoA-I与GAGS评分之间仍存在显著的相关性(r=0.511,P<0.001)。结论 ApoB/ApoA-I升高可能加重PCOS患者痤疮程度,两者之间的相关性值得进一步探究。 Objective To explore the effect of the ApoB/ApoA-I ratio to the acne degree in PCOS patients. Methods 115 patients with PCOS treated in Zigong Maternal and Child Health Hospital from June to August 2018 were selected as the research subjects. Patients were graded according to the gross acne grade system(GAGS) for the extent of their skin lesions. The differences of blood lipid, sex hormone, insulin resistance, ApoB/ApoA-I were compared among patients with different acne degrees. The correlation between ApoB/ApoA-I and GAGS scores and patients′ metabolic and endocrine clinical parameters was analyzed. Results The degree of acne in PCOS patients was correlated with the levels of insulin resistance index(HOMA-IR), body mass index(BMI), testosterone(T), ApoB/ApoA-I and blood lipid. After adjusting for the contributions of insulin resistance, obesity and T, a significant association remained between ApoB/ApoA-I and GAGS score(r=0.511, P<0.001). Conclusions The increase of ApoB/ApoA-I may aggravate the degree of acne in PCOS patients, and the correlation between the two is worthy of further exploration.
