原发性干燥综合征患者舌象特点及其与证候的相关性研究

Tongue appearances of patients with primary Sjögren’s syndrome and their correlations with syndromes

目的:总结原发性干燥综合征(pSS)患者舌象特点,分析舌象与证候的相关性。方法:纳入2018年12月至2019年12月于中日友好医院中医风湿病科就诊的pSS患者,统一采集舌象并辨证,卡方检验比较不同证候要素的舌象特点,Spearman相关分析舌象与证候的相关性。结果:共纳入84例pSS患者,常见证候要素包括阴虚、气虚、血瘀和痰湿证。阴虚证常见舌体瘦薄、裂纹、舌色红/绛、无苔,舌象与证候呈正相关;气虚证常见无苔,二者呈正相关;血瘀证常见舌色暗红/紫暗,二者呈强相关;痰湿证常见舌苔厚、腻,相关性为强相关。结论:pSS患者舌体瘦薄、裂纹、舌色红/绛与阴虚证相关,无苔与阴虚、气虚证相关,舌色暗红/紫暗与血瘀证强相关,舌苔厚、腻与痰湿证强相关。

Objective:To summarize the characteristics of tongue appearances of patients with primary Sjögren’s syndrome(pSS)and analyze their correlation with traditional Chinese medicine(TCM)syndromes.Methods:Patients with pSS who visit TCM department of rheumatism in China-Japan Friendship Hospital were considered for enrolling in this study,from December,2018 to December,2019.Tongue appearances and TCM syndromes were uniformly collected.Chi-square test were used to analyze the features of tongue appearances for patients with major syndrome elements.Spearman correlation analysis were used to test the correlation between tongue appearances and major syndrome elements.Results:A total of 84 pSS patients were included in this study.Yin deficiency,qi deficiency,blood stasis and phlegm dampness were major syndrome elements in pSS patients.Patients with yin deficiency syndrome often had thin tongue,cracked tongue,red or deep-red tongue and no coating,these tongue appearances were positively correlated with yin deficiency syndrome;patients with blood stasis often had dark red tongue or dull purple tongue,there was a strong correlation between them;patients with qi deficiency syndrome often had no coating,they were positively correlated;patients with phlegm-damp syndrome often had thick coating and greasy coating,there were strong correlations among them.Conclusion:pSS patients with thin tongue,cracked tongue,red or deep-red tongue are positively correlated with yin deficiency syndrome,no coating is positively correlated with yin deficiency and qi deficiency syndrome,dark red tongue or dull purple tongue has strong correlation with blood stasis syndrome,while thick coating and greasy coating had strong correlation with phlegm-damp syndrome.
