止痛散结涂剂外用联合远红外线照射治疗动静脉内瘘皮下血肿的疗效观察

Observation of curative effect of Zhitong Sanjie Ointment external treatment combined with far-infrared light on subcutaneous hematoma of arteriovenous fistula

目的:观察外用中药止痛散结涂剂治疗动静脉内瘘皮下血肿的疗效。方法:将50例血液透析治疗过程中出现动静脉内瘘皮下血肿的患者,按其血肿的出现顺序,随机分为治疗组与对照组,各25例。皮下血肿处均给予按压止血、冰敷等常规处理,24h后治疗组予以止痛散结涂剂外涂联合远红外线照射,对照组采用多磺酸粘多糖乳膏外涂联合远红外线照射,疗程均为2周。比较两组血肿消退时间、治疗1d后疼痛程度评分、是否出现皮下硬结情况、动静脉内瘘血肿处血管壁厚度及其所出现的不良反应。结果:与对照组比较,治疗组血肿消退时间、1d后疼痛评分、穿刺处血管壁厚度及出现皮下硬结的例数均显著降低(P<0.01,P<0.05),且两组均未见明显不良反应。结论:中药外用止痛散结涂剂在促进动静脉内瘘局部血肿消退、改善血管条件及缓解患者疼痛等方面疗效明显。

Objective:To observe the effects of Zhitong Sanjie Ointment on subcutaneous hematoma of arteriovenous fistula.Methods:According to the appearance of hematoma in order,the 50 patients who get hematogenous during hemodialysis process were randomly divided into treatment group and control group,25 cases in each group.Every hematoma were given compressionhemostasis and ice compress.After 24 hours,the treatment group was treated with external application of Zhitong Sanjie ointment in combination with far-infrared light,while the control group received xiliaotuo combined with far-infrared irradiation,the course of the treatment was two weeks.Results:The time of hematoma subsidence,the pain score after 1 day,the thickness of the vascular wall at the puncture site,the number of cases with subcutaneous hard nodules,and adverse reactions were compared.Results:In the treatment group,the time of hematoma subsidence,the pain score after 1 day,the thickness of the vascular wall at the puncture site and the number of cases with subcutaneous hard nodules were significantly decreased(P<0.01,P<0.05),and no obvious adverse reactions were observed in the two groups.Conclusion:The external application of Zhitong Sanjie Ointment has obvious curative effect in promoting the regression of local hematoma in arteriovenous fistula,improving vascular conditions and relieving patients’pain.
