摘要 目的:探讨纤维支气管镜肺泡灌洗辅助治疗儿童重症肺炎的效果及对患儿血清学感染指标及血气指标的影响。方法:选择2018年2月-2020年8月笔者所在科室收治的重症肺炎患儿62例,分为观察组32例和对照组30例。对照组患儿从入院至行纤维支气管镜治疗时间>3 d;观察组患儿从入院至行纤维支气管镜治疗时间≤3 d。观察并分析两组患者治疗总有效率,观察血清学感染指标及血气指标改善情况,记录治疗后热程时间和住院天数情况。结果:采用纤维支气管镜治疗患儿后,观察组有效率(93.75%)优于对照组(73.33%),差异有统计学意义(P<0.05);观察组血气改善情况优于对照组,差异有统计学意义(P<0.05);观察组CRP恢复时间、WBC恢复时间、热程及住院天数显著短于对照组,差异有统计学意义(P<0.05)。结论:纤维支气管镜早期介入治疗重症肺炎患儿,可以提高治疗有效率,显著改善患儿血气情况,缩短CRP恢复时间、WBC恢复时间、热程及住院天数。 Objective:To investigate the effect of bronchoalveolar lavage with fiberoptic bronchoscope in the treatment of children with severe pneumonia and its influence on serological infection index and blood gas analysis.Method:A total of 62 children with severe pneumonia in our department from February 2018 to August 2020 were selected and divided into observation group (32 cases) and control group (30 cases).The control group from admission to fiberoptic bronchoscopy treatment time>3 days;the observation group from admission to fiberoptic bronchoscopy treatment time≤3 days.Observed and analyzed the total effective rate of the two groups,observed the improvement of serological infection index and blood gas,record the heat course time and hospitalization days after treatment.Result:After the treatment of children with fiberoptic bronchoscopy,the effective rate of the observation group (93.75%) was better than that of the control group (73.33%),the difference was statistically significant (P<0.05);the improvement of blood gas of the observation group was better than that of the control group,the difference was statistically significant (P<0.05);the recovery time of CRP,WBC,heat course and hospitalization days of the observation group were significantly shorter than those of the control group,the difference was statistically significant (P<0.05).Conclusion:Early interventional therapy with fiberoptic bronchoscopy in children with severe pneumonia can improve the treatment efficiency,significantly improve the blood gas status of children,shorten the recovery time of CRP,WBC,fever and hospitalization days.
