自拟益气润肠丸内服联合玄明粉熏洗治疗脱垂性痔病的临床观察

Clinical Observation of Using Self-made Runchang Pills Combined with Mirabilitum Dehydratum Fumigation in the Treatment of Prolapsed Hemorrhoids

目的:观察自拟益气润肠丸内服联合玄明粉熏洗治疗脱垂性痔病的临床疗效。方法:收集2017年11月~2020年10月我院收治的85例脱垂性痔病(脾虚气陷型)患者作为研究对象,采用随机数字表法分为联合组45例,熏洗组40例,熏洗组给予玄明粉熏洗治疗,联合组在熏洗组的基础上给予自拟益气润肠丸内服。治疗15d后对比两组患者临床疗效、中医症状积分、视觉模拟评分(VAS)、肛门功能评分、恢复速度。结果:治疗后联合组总有效率88.89%明显高于熏洗组70.00%(P<0.05);治疗后联合组排便费力、痔核脱出、便血、肛门下坠、舌象、脉象等症状积分明显低于熏洗组上述主要症状积分(P<0.05);治疗后联合组患者VAS评分[(0.74±0.26)分]低于熏洗组VAS评分[(2.01±0.47)分](P<0.05);治疗后联合组患者肛门功能评分感觉功能[(2.46±0.37)分]、控便功能[(2.86±0.24)分]、闭合功能[(2.64±0.51)分],明显高于熏洗组(P<0.05);治疗后联合组患者排便费力[(8.21±1.67)d]、痔核脱出[(6.05±1.34)d]、便血[(8.13±2.04)d]、肛门下坠[(5.16±1.64)d]恢复时间明显短于熏洗组(P<0.05)。结论:自拟益气润肠丸内服联合玄明粉熏洗治疗脱垂性痔病疗效良好,值得临床应用。

Objective: To explore the clinical efficacy of using Self-made Runchang Pills combined with Xuanming Powder(Mirabilitum Dehydratum) fumigation in the treatment of prolapsed hemorrhoids. Methods: A total of 85 patients with prolapsed hemorrhoids(spleen deficiency and Qi collapse) admitted to our hospital from February 2017 to October 2020 were collected as the research subjects, and they were divided into a combined group(45 cases) and a fumigation group(40 cases) according to the random number table method. The fumigation group was given Mirabilitum Dehydratum fumigation, and the combined group was given Self-made Runchang Pills on the basis of the fumigation group. After 15 days of treatment, the clinical efficacy, TCM symptoms scores, visual analogue scale(VAS),anal function scores and recovery speed were compared between the two groups. Results: After treatment, the total effective rate in the combined group was significantly higher than that in the fumigation group(88.89% vs 70.00%)(P<0.05). After treatment, the scores of symptoms such as laborious defecation, prolapse of hemorrhoids, hematochezia, anus drop, tongue and pulse in the combined group were significantly lower than the above-mentioned main symptoms scores in the fumigation group(P<0.05). After treatment, the VAS score in the combined group was lower than that in the fumigation group [(0.74±0.26) points vs(2.01±0.47) points](P<0.05). After treatment, the anal function scores of sensory function, bowel control function and closure function with(2.46±0.37) points,(2.86±0.24) points and(2.64±0.51) points in the combined group were significantly higher than those in the fumigation group(P<0.05). After treatment, the recovery times of laborious defecation, prolapse of hemorrhoids, hematochezia and anal drop with(8.21±1.67) d,(6.05±1.34) d,(8.13±2.04) d and(5.16±1.64) d in the combined group were significantly shorter than those in the fumigation group(P<0.05). Conclusion: Self-made Runchang Pills combined with Mirabilitum Dehydratum fumigation has a good efficacy on prolapsed hemorrhoids, and it is worthy of clinical application.
