摘要 目的目前超声全身动脉僵硬度自动测量系统(AMAS)测量高血压患者动脉脉搏波速度的研究较少。文中旨在探讨基于AMAS测得的颈-股动脉脉搏波速度(cfPWV)值用于检测高血压患者动脉弹性的临床价值。方法回顾性分析2019年11月至2020年11月东部战区总医院心血管内科住院的83例高血压患者为高血压组,同期从东部战区总医院体检中心选择85例健康体检者作为对照组。应用同一超声诊断机AMAS测量受试者的cfPWV和颈动脉内中膜厚度(IMT)。将高血压组的83例患者分别按照血压(轻度、中度、重度)、脉压(<60 mmHg、60 mmHg≤脉压<75 mmHg、≥75 mmHg)分层,比较各组IMT、脉压、cfPWV等参数。结果高血压组IMT、cfPWV[(0.71±0.16)mm、(8.33±1.83)cm/s]明显高于对照组[(0.62±0.20)mm、(7.21±1.44)cm/s],差异有统计学意义(P<0.05)。轻度、中度、重度高血压患者的cfPWV随着血压水平的升高而增快,两两比较差异均有统计学意义(P<0.05)。与脉压<60 mmHg和60~75 mmHg比较,脉压≥75 mmHg的IMT和cfPWV显著增高(P<0.05)。收缩压、脉压是cfPWV的影响因素(P<0.05)。结论AMAS检测cfPWV有助于早期发现高血压患者动脉功能的改变,对临床早期干预具有重要意义。 Objective Few studies have been reported on the measurement of arterial stiffness in patients with hypertension by the ultrasonic Automatic Measurement of Arterial Stiffness(AMAS)system.This study was to investigate the clinical value of the carotid-femoral pulsewave velocity(cfPWV)measured by the AMAS system in detecting the arterial elasticity in hypertension patients.Methods From November 2019 to November 2020,83 patients with hypertension were selected from the General Hospital of Eastern Theater Command as the study group.During the same period,85 healthy people from the physical examination center of our hospital were selected as the control group.The same ultrasound diagnostic machine was deployed to measure the carotid artery intima-media thickness(IMT)and cfPWV.Such parameters as IMT,pulse pressure and cfPWV of the study group and the control group were compared,and 83 patients in the study group were stratified according to blood pressure(grade 1,2 and 3)and pulse pressure(<60 mmHg,60 mmHg≤pulse pressure<75 mmHg,≥75 mmHg).Results The IMT and cfPWV of the study group were(0.71±0.16)mm and(8.33±1.83)cm/s,while the IMT and cfPWV of the control group were(0.62±0.20)mm and(7.21±1.44)cm/s.The IMT and cfPWV of the study group were significantly higher than those of the control group(P<0.05).The cfPWV increased with the rise of blood pressure in patients with hypertension grade 1,2 and 3.And the differences between the groups were statistically significant(P<0.05).Compared to pulse pressure<60 mmHg and 60-75 mmHg,IMT and cfPWV of pulse pressure≥75 mmHg were significantly increased(P<0.05).Multiple linear regression analysis showed that SBP and PP(P<0.05)were independent factors of cfPWV.Conclusion AMAS detection of cfPWV is conducive to find the changes of arterial function in patients with hypertension and is of significance for early clinical intervention.
