磁共振成像全容积表观扩散系数直方图鉴别乳腺浸润性导管癌与纤维腺瘤的价值

The value of histogram of all-solvent apparent diffusion coefficient of magnetic resonance imaging in distinguishing breast invasive ductal carcinoma and fibroafenoma

目的探索磁共振成像(MRI)表观扩散系数(ADC)直方图分析在乳腺浸润性导管癌与纤维腺瘤中的价值。方法回顾性分析于我院取得病理结果并在术前行乳腺MRI检查的55例乳腺浸润性导管癌、43例乳腺纤维腺瘤患者的ADC图像。导入MaZda软件通过ADC图像的灰度分布情况和像素值绘制直方图并提取直方图相关9个参数。对浸润性导管癌组、纤维腺瘤组的各直方图参数进行比较,并运用受试者工作特征曲线(ROC)分析直方图各参数鉴别乳腺浸润性导管癌与纤维腺瘤的诊断效能。结果浸润性导管癌组的均值、第99、90、50、10、1百分位数等6个直方图参数均低于纤维腺瘤组(P均<0.01),而浸润性导管癌组的偏度高于纤维腺瘤组(P<0.001),其中均值第50百分位数具有最高的诊断效能,AUC为0.96,敏感度为83.7%,特异度为91.2%。结论MRI的ADC直方图参数有助于鉴别乳腺浸润性导管癌与纤维腺瘤,具有较大诊断价值,对临床工作有较大指导意义。

Objective To explore the value of magnetic resonance imaging(MRI)apparent diffusion coefficient(ADC)histogram analysis in breast invasive ductal carcinoma and fibroadenoma.Methods The ADC images of 55 cases of breast invasive ductal carcinoma and 43 cases of breast fibroadenoma were analyzed retrospectively.These images were imported into Mazda software,which draws histogram through gray distribution and pixel value of ADC image,and extracts nine parameters related to histogram.The histogram parameters of invasive ductal carcinoma group and fibro-adenoma group were compared.Receiver operating characteristic(ROC)curve was used to analyze the diagnostic efficiency of histogram parameters in differentiating invasive ductal carcinoma from fibroadenoma.Results The histogram parameters(mean,Perc.99%,Perc.90%,Perc.50%,Perc.10%and Perc.1%)of invasive ductal carcinoma group were lower than those of fibroadenoma group(P<0.001),while the skewness of invasive ductal carcinoma group was higher than that of fibroadenoma group(P<0.001).The 50th percentile of mean had the highest diagnostic efficiency,AUC was 0.96,sensitivity was 83.7%,and specificity was 91.2%.Conclusion The ADC histogram parameters of MRI are helpful to differentiate breast invasive ductal car-cinoma and fibroadenoma,which has great diagnostic value and has great guiding significance for clinical work.
