腹腔镜精索静脉高位结扎术治疗精索静脉曲张的临床研究

Clinical Study of Laparoscopic High Ligation of Spermatic Vein in the Treatment of Varicocele

目的:探讨腹腔镜精索静脉高位结扎术治疗精索静脉曲张效果。方法:将100例精索静脉曲张患者随机分为对照组和观察组各50例,对照组行经腹膜外开放性精索静脉高位结扎术,观察组行腹腔镜精索静脉高位结扎术,比较两组围术期指标、疼痛程度[视觉模拟评分法(VAS)]。结果:观察组总有效率高于对照组(P<0.05),两组手术时间比较差异无显著性(P>0.05);观察组术中出血量少于对照组,住院时间短于对照组(P<0.01),术后1 d、3 d的VAS评分均低于对照组(P<0.01)。结论:腹腔镜精索静脉高位结扎术用于精索静脉曲张的治疗中,改善手术效果,可降低术后疼痛,缩短康复时间。

Objective:To investigate the effect of laparoscopic high ligation of spermatic vein in the treatment of varicocele.Methods:100 patients with varicocele were divided into two groups,with 50 cases in each group.The control group was treated with open high ligation of spermatic vein by extraperitoneal approach,and the observation group was treated with laparoscopic high ligation of spermatic vein.The perioperative indexes and pain degree(VAS)between the two groups were compared.Results:After operation,the total effective rate in the observation group was higher than that in the control group(P<0.05).There was no significant difference in operation time between the two groups(P>0.05).Compared with the control group,the observation group had less intraoperative blood loss and shorter hospital stay,and the difference was statistically significant(P<0.01).Compared with the control group,the VAS scores of the observation group were lower on 1d and 3d after operation,and the difference was statistically significant(P<0.01).Conclusion:Laparoscopic high ligation of spermatic vein in the treatment of varicocele can improve the surgical effect,reduce postoperative pain and shorten the recovery time.
